One fatality, one serious According to Sheriff's Office, a/c crashed into cotton field 500 feet N of departure end of runway 32. No eyewitnesses were located.
Sept 08, 2009 Commune de Brethencourt, France Reims F172 F-BXIU G-DANT A/c collided in mid-air near Commune de Brethencourt, France. Two fatalities Visual meteorological conditions prevailed at time of accident. Pilot and one passenger in G-DANT not injured. Sudent pilot and flight instructor aboard F-BXIU fatally injured.
Sept 08, 2009 Los Angeles, CA BOEING 737 N670SW A/c operated by Southwest Airlines as flight 341, experienced fracture of inboard axle on right main landing gear during landing on runway 24R at Los Angeles International (LAX). No injuries RMLG inboard wheel/brake assembly separated from a/c and recovered on N side of runway near 4,000 feet remaining marker. A/c taxied off runway to taxiway Y where passengers were deplaned by air stairs.
Sept 11, 2009 BOMBARDIER DHC-8-202 N367PH On unknown date and at unknown time, a/c was substantially damaged when bottom of tail section damaged due to unknown reasons. Damage found during maintenance C-check inspection on Aug. 29, 2009. Skin, rivets, and center former of bulkhead damaged. Damaged section located between stations 545.00 and 600.00 with concentrated damage located between stations 565.50 and 580.50 along longitudinal axis and between stations 31 right hand and 31 left hand along the lateral axis on the fuselage. unavail A/c operated by Champlain Enterprises (CommutAir), a Continental Airlines connection carrier Damage was consistent with tail strike damage and had resulted in a puncture of the pressure vessel of a/c. Damage found when maintenance personnel inspecting inside of tail section and noticed bowing and bending in support structure and damage to nearby rivets. Further inspection of exterior of a/c revealed skin damage.
Sept 17, 2009 Winter Haven, FL CURTIS JOHN P SEA REY N563JL Amphibious a/c, registered to and operated by an individual, crashed into Lake Otis. Two serious Witness observed a/c landing on water from S. A/c taxied and turned around near N end of lake and pointed itself for southerly departure. A/c lifted off and everything appeared normal until a/c reached altitude 100 feet above the water. A/c dipped to left and then it seemed to right itself to the right as it entered the water flat.
Sept 18, 2009 Coffeyville, KS MCCLISH FUNK B85C N81167 A/c destroyed following cockpit fire during initial climb and subsequent successful landing at Coffeyville Municipal Airport (CFV). No injuries The airplane was registered to and operated by a private individual.
Sept 20, 2009 Lowndesville, SC CESSNA 172 N5845A A/c substantially damaged after impacting waters of Lake Russell. Witnesses said a/c was heard flying overhead of their location several times. One of the witnesses reported that the engine was "making a pop pop sound intermittently" then it sounded like a lot of engine power was applied. Two fatalities They then observed a/c descending out of "very low" overcast cloud layer between 70-90 degrees nose down attitude with wings level on path directly toward them. It veered away from their location, and then impacted water75 feet from their boat.
Sept 23, 2009 Hilltop Lakes, TX CESSNA T210 N9734Y A/c crashed into unoccupied residence while executing a go around during landing attempt at Hilltop Lakes Airport (0TE4). Runway lights at 0TE4 inoperative following recent lightning strike and pilot was attempting to line up on runway using runway edge reflectors. One witness received an in-flight cellular telephone call from the pilot and parked his car with the headlamps illuminating the runway. One fatality Pilot acknowledged seeing car lights on runway. Pilot then flew eastbound over airport and attempting a right turn to land on runway 34. Several witnesses said pilot started a go-around at approximately 50-150 feet agl, but almost immediately started a descent.
Sept 24, 2009 Houma, LA CESSNA 182 N96984 A/c impacted terrain following flight into instrument meteorological conditions shortly after takeoff from Houma-Terrebonne Airport (HUM). One fatality Accident site located one mile S of departure end of Runway 18 and two tenths of a mile W of extended runway centerline. Examination of a/c wreckage and accident site indicated a/c impacted trees and terrain in 35-degree left bank and 30 degrees nose low.
Sept 24, 2009 Lumberton, MS DUKE PAUL Challenger II N4340D A/c substantially damaged when it impacted trees and terrain near Lumberton One fatality During landing attempt, witness did not hear engine power up. Left main landing gear then struck tree top, and a/c banked hard to left. A/c then entered "nose dive".
Sept 27, 2009 Vineyard Haven, MA BOMBARDIER CL-600-2B16 N190MP A/c damaged while landing at Martha's Vineyard Airport (MVY). No injuries Flight crew said when a/c 15-20 feet above runway, encountered windshear which resulted in loss of 25 knots indicated airspeed. A/c landed hard and bounced on main landing gear and nose landing gear.
Sept 27, 2009 Myrtle Creek, OR ARROW FALCON EXPORTERS OH-58A+ N298CP A/ collided with trees during takeoff from a field landing zone, 24 miles W of Myrtle Creek.. Reforestation Services operated a/c. Purpose of flight was to survey area to be sprayed. As a/c departed landing zone pilot sensed that more power than usual was being required to clear 10-12-foot trees at edge of LZ. One minor Checked engine instruments, which indicated normal readings, and 100 percent torque. Once over trees, between 20-30 knots airspeed, pilot said a/c would not accelerate and settled into the trees despite applying full collective.
Sept 28, 2009 Wasilla, AK HUGHES 369A (OH-6A) N501SU A/c sustained substantial damage following loss of engine power and subsequent emergency landing during takeoff from Wolf Lake Airport. Pilot said during takeoff, as a/c climbed to 35 feet agl, it suddenly yawed to right, No injuries the red engine out annunciator light illuminated, and low rotor warning horn sounded. Pilot entered emergency autorotation and attempted to cushion landing, but a/c landed hard and main rotor blades struck tail boom, severing it from a/c.
Sept 30, 2009 Albany, IN Mooney M20M N400DE A/c destroyed during post-impact fire after impacting trees and terrain near Albany. One fatality Pilot became unresponsive during flight and a/c was subsequently intercepted by Air National Guard F-16s, which tailed the a/c until it crashed in a cornfield near Albany.
Oct. 6, 2009 Elmendorf AFB, AK U.S. Army Citation-560 Pilot of Army Citation-560forced to make gear up landing at Elmendorf AFB. Base emergency officials responded. Three people were on board a/c, but no one was injured in the accident. No injuries Cause of the accident is unknown at this time pending an investigation.

Two fatalities, serious injuries Plane was on demonstration flight at air show at Maatiga military airport when it crashed in the nearby Soug-Ejemaa district.
Oct. 8, 2009 Dayton, OH replica Wright brothers' 1905 Wright Flyer III Pilot of replica Wright Flyer critically injured in crash during practice flight for re-enactment of historic flight on Wright-Patterson AFB. One critical Pilot and vintage aircraft builder Mark Dusenberry, 48, had completed one practice flight and was on a second when the replica plane crashed,.
Oct. 9, 2009 Fonds-Parisienn, Haiti CASA C-212 All onboard died after twin turboprop ai/c went down near border with Dominican Republic, about 30 miles from Haitian capital of Port-au- Prince. 11 fatalities Victims were Uruguayan and Jordanian troops serving with U.N. peacekeeping force. U.N. statement said a/c on "regular reconnaissance flight."
Oct. 9, 2009 Marathon, FL unvail Three persons survived a/c ditching in Gulf of Mexico by clinging to buoy five miles from southern tip of Minor injuries mainland Florida for more than 12 hours. A/c lost all power.

Oct. 10, 2009 Pineville, LA Cessna 150s Two a/c at recreational pilots gathering in central Louisiana collided in the air, killing two and injuring two. Two fatalities, two serious FAA said each Cessna 150 carried two people when they hit and crashed into woods near Pineville Regional Airport.
