Instructor positioned himself at cabin door opening facing inside the cabin, while skydiver was immediately in front of him. | One fatality | Instructor's reserve chute deployed pulling instructor from a/c. Controller approved a heading change and during turn first officer noticed weather directly in aircraft's path. First officer continued turn additional 30 degrees to avoid weather, but flight encountered moderate turbulence that lasted approximately 15 seconds. | One serious | F/A was in process of stowing bev cart when turbulence occurred. Became airborne along with bev cart and when landed heard a "crunch" type sound. Crawled to intercom phone and alerted captain that she was injured. Two passengers assisted F/A by securing bev cart, and F/A made all required safety announcements. Emergency was declared with ATC and flight proceeded to EWN and landed without further incident. |
| August 07, 2009 Winchester, VA | PIPER PA-28-161 N20977 | A/c substantially damaged during forced landing near Winchester Regional Airport (OKV). While flying 3-5 miles from airport, engine went from "full" power to "idle" power. Pilot lowered airplane's nose and broadcasted the situation via OKV common traffic advisory frequency. Pilot attempted to remedy situation by switching from left to right fuel tank, and cycled throttle from full, to idle, and back to full. Also cycled mixture control in similar fashion. About 3-5 seconds later engine returned to full power, but only for several seconds, before returning back to idle. | One minor | Pilot repeated actions two additional times, with same response from the engine. Pilot also continued to position a/c for landing on runway 32 at OKV as attempted to troubleshoot engine problem. Pilot realized that a/c would not be able to make runway, so performed forced landing to left side of approach lighting system. A/c struck a berm and fence during landing. |

Witnesses stated to responding FAA inspector that this was pilot/owner's initial flight with a/c. Witnesses observed a/c departing toward S, where the terrain ascended. Witnesses advised that normal departure procedures for grass strip was to depart toward N, where terrain descended. | One fatality | A/c was observed to make normal takeoff roll. A/c became airborne, pitched up while maneuvering, and then went straight down. A/c impacted nose first, coming to rest on its back side, adjacent to right side of grass strip; a fire ensued immediately. A/c's engine was heard at full power until ground impact. |

The intended destination was O'Neill Municipal Airport (ONL). Flight's ultimate destination was Napoleon, ND, with intermediate stop at ONL to refuel. A/c reported overdue by operator. | One fatality | Local authorities notified of possible downed aircraft on Aug. 16. Sheriff's Department and State Patrol efforts to find a/c during night were unsuccessful. A/c was subsequently located after daylight aerial search. A/c impacted open field about 8 miles E of Corning. Debris path was oriented on easterly course, and 500 feet long. A/c was fragmented during impact sequence. |
| August 15, 2009 Spartanburg, SC | CESSNA 180 N1044Y | A/c operated by South Carolina Forestry Commission substantially damaged when struck standing Beech J35, N4398D, while taxiing at Spartanburg Downtown Memorial Airport (SPA). Cessna had landed and taxied off runway toward hangar area. Pilot continued along taxiway, looking out side windows as taxied. When turned right onto taxiway that led directly to hangar area, felt a "thump" and saw man's face through left window. He immediately shut down engine, exited a/c , and saw that Cessna's propeller had torn into t Beech's left wing fuel tank. | No injuries | Beech pilot had just completed changing oil and was performing extended run-up prior to departing on brief local flight. Parked near edge of ramp area, and believed sufficient room available for a/c to exit ramp or taxiway without difficulty. Beech pilot noticed "tail dragger coming down taxiway fairly fast." As Cessna approached intersection of taxiway and ramp area where Beech was parked, Cessna "came whipping around the corner" and struck Beech. | August 16, 2009 Hesperia, CA | CESSNA 152 N67361 | A/c descended into terrain about 1/4-mile SE of Hesperian Airport. Witness near airport reported to Safety Board investigator that observed a/c make a high and fast approach to runway 21. Unable to land, the airplane's pilot made go around. | Two fatalities | A/c returned for second approach to runway 21, which was again flown high and fast. Thereafter, pilot initiated another go-around. Witness described a/c's climb and left low altitude crosswind leg turn into traffic pattern as being unusual, in that airplane's pitch attitude was upward about 45 degrees, and airplane's left bank angle was about 60 degrees. Also, a/c turned left for left-hand traffic pattern very soon after over flying departure end of runway 21. |

She remembered turning on search light and shortly after, impacting water. She said she thought she "pulled power and cyclic" when she saw the water; but didn't have time to warn the crew.
