Sept 02, 2009 Kurnoon, India BELL 430 VT-APG Government of Andhra Pradesh Gad helicopter crashed enroute to Chitoor, India from Hyderabad, India. There were 2 crew members and 3 passengers, including the Secretariat of Hyderabad on board. Five fatalities The helicopter was destroyed and there were no survivors. The investigation is being conducting by Central Government of India.
Sept 05, 2009 Tulsa, OK PIPER PA-32R-300 N1228H A/c struck radio tower guide wire and subsequently struck ground 8 miles NW of Richard Lloyd Jones, Jr./Riverside Airport (KRVS). Five fatalities A 7-strand steel guy-wire supporting the tower on its W side and located approximately 150 feet up from tower's base, severed 50 feet out from where wire attached to tower. 300-foot section of wire located with a/c main wreckage 1,575 feet W of tower.
Sept 06, 2009 Boca Raton, FL BLUM MATTHEW AVENTURA I N6301M Experimental a/c experienced in-flight loss of control and crashed about 1 mile N-NW of departure end of runway 05 at Boca Raton Airport (BCT). One fatality Witnesses reported seeing wings of a/c rocking, a steep turn, followed by the nose pitching down.
Sept 17, 2009 Newark, NJ BOEING 767 N66051 A/c experienced fracture of left main landing gear truck beam while a/c was stopped on taxiway D with parking brake set prior to takeoff. No injuries A/c operated by Continental Airlines from Newark, NJ to Frankfurt, Germany. A/c engines were shut down and passengers taken off by airstairs.
Sept17, 2009 Copenhagen, Denmark AIRBUS A319 G-EUPL British Airways A319-131 powered by two IAE V2522-A5 engines experienced bird strike and subsequent engine stall in No. 1 engine (left-hand) during climb after takeoff at 5,000 feet. unavail In-flight engine shutdown performed and a/c returned to Copenhagen Airport (CPH) for uneventful single-engine landing. Engine removed and shipped to Rolls-Royce East Kilbride engine facility in Scotland for disassembly and examination.
Sept 25, 2009 Barbones, Ecuador PIPER PA18 HC-AHD A/c operated by FumiPalma S.A., crashed following loss of engine power, while on approach for landing at Pista Bardones, El Oro, Two fatalities Pilot/passenger were killed and a/c was destroyed by post-crash fire. Flight originated from same airstrip 10 minutes prior to accident.
Oct 02, 2009 Sulzberg, Germany CESSNA F172N D-EDVG A/c destroyed when impacted terrain under unknown circumstances near Sulzberg One fatality Pilot fatally injured. Flight originated from St. Johann, Austria, and was en route to Michelstadt, Germany.
Oct 04, 2009 Black Canyon City, AZ PIPER PA-24-250 N7471P A/c destroyed when collided with terrain near Black Canyon City. On Oct. 5, USFS firefighters reported locating wreckage of downed airplane while fighting a wildfire near accident site. Two fatalities Examination of accident site by NTSB investigator-in-charge revealed that a/c impacted terrain on eastern slope of mountain ridge. All major structural components of a/c were present at accident site. Fuselage was mostly consumed by post-impact fire.
Oct 05, 2009 Yankton, OR Turpen RV-8 N61FS A/c impacted terrain. Pilot flying under VFR and had not filed flight plan. As approached destination, IMC prevailed and he requested an IFR clearance. One serious The pilot was cleared for ILS approach to runway 15. Shortly thereafter, a/c radar target no longer appearing on radar.
Oct 08, 2009 Burlington, WA PIPER PA-12 N2087C A/c mpacted terrain after lifting off from Runway 28 at Skagit Regional Airport. Witnesses said pilot pulled onto runway 28 added full power and after rolling 200 feet, the airplane lifted off and climbed to near vertical attitude One fatality After reaching height of 200 feet, a/c rolled off on right wing, descended near vertically into flat grassy terrain between crossing runway and its adjacent taxiway.
Oct09, 2009 Phoenix, AZ Cruz SE5A Replica N3944U Bi-plane crashed into scrap metal yard about half-mile N of airport shortly after takeoff from Phoenix Deer Valley Airport. One fatality Witnesses saw a/c begin right turn, when "wing went over." A/c entered spin and collided with ground in nose-down attitude.
Oct10, 2009 Ocala, FL BELL OH-58A N911AF Bell OH-58A operated by Marion County Sheriff's Office was substantially damaged following hard landing at Ocala International Airport (OCF). Pilot said while on final approach for landing, about 120 feet altitude, felt a "vibration" and a/c experienced loss of engine and rotor rpm. No injuries Pilot lowered collective for immediate landing and heard low-rotor warning horn during descent. A/c experienced hard landing, and rotated to left after touchdown. Once a/c "settled," pilot closed the throttle and exited a/c.
Oct 12, 2009 Fossil, OR MCDONNELL DOUGLAS HELI COPTER 369FF N527SH A/c collided with wood power pole about 18 miles SE of Fossil. Witnesses said a/c was placing a rope, which would later be used to pull a power line onto pulleys that were One fatality mounted on the power poles. During accident sequence, one or more of the helicopter's main rotor blades came in contact with power pole.
Oct 12, 2009 San Diego, CA MD HELICOPTER 600 N613BP A/c landed hard following practice 180-degree autorotation at Gillespie Field Airport (SEE). DHS CBP operated a/c as a public-use training flight. McDonnell Douglas factory pilot and CBP standardization pilot not injured. No injuries A/c sustained structural damage during hard landing as skids spread and aft portion of skids penetrated fuselage.
Oct 12, 2009 Corsisa unavail French authorities rescued six people who survived in Mediterranean Sea for more than five hours after their small plane crashed off coast of Corsica. Serious injuries A/c flying from Propriano in southern Corsica to Cannes French Riviera when crashed in the Gulf of Porto. Pilot reported engine failure to ATC and announced would try to land in the sea one hour after takeoff.

Oct. 14, 2009 Tahlequah, OK ROBINSON R22 BETA A/c operated by private pilot destroyed when impacted terrain near Tahlequah. A/c impacted open field, adjacent to small stream on magnetic heading of 105 degrees and field elevation of 800 feet msl, one mile SE of departure location. Two fatalities Debris scattered for 220 feet. Main wreckage consisted of main rotor blades, mast and transmission, fuselage, engine and tail boom assembly.
Oct. 15, 2009 South Carolina F-16 fighters Two F-16 jets collided near the South Carolina coast during a night-training exercise, One fatality On pilot able to land safely at Charleston AFB, SC. Debris found of second F-16 by USCG.
Oct 17, 2009 3,5 km SW of Manila-Ninoy Aquino International Airport (MNL) Philippines Douglas DC-3C (C-47B-1-DK) RP-C550- DC-3 operated by Victoria Air destroyed when crashed into warehouse shortly after takeoff from Manila-Ninoy Aquino International Airport. A/c took off from runway 24. Four fatalities A/c reportedly carried a cargo consisting of six drums of aircraft fuel on a domestic flight to Puerto Princesa Airport (PPS). Shortly after takeoff the crew requested for emergency landing after aircraft reportedly experienced engine problems.

Oct 21, 2009 1 km N of Sharjah Airport (SHJ) UAE Boeing 707-330C ST-AKW? Boeing 707 cargo plane, owned by Azza Transport destroyed when crashed in desert area immediately after takeoff from Sharjah Airport (SHJ). Operated as Sudan Airways Flight SUD2241. Six fataliies Eyewitnesses reported seeing a/c in excessive right bank after takeoff. Boeing 707 crashed near the Emirates Road, broke up and burned.
