26 Aug. 0100Z Winnipeg Manitoba 737-281 of WestJet C-FTWJ Flt: WJA409 1500ft into climb-out & eight CB's popped, both gens remained online but Wx Radar, #2 Nav and #2 comm lost. Nil Ret'd Winnipeg. #2 bus-tie CB replaced. After 84 cycles sequence repeated. #2 gen had open coil (replaced).
21 Sept. Toronto Canada 767-333 of Air Canada C-FMWV Crew of Flt 891 enrt Rome-Toronto noted electric smell and got EICAS fwd equipment exhaust fan message. Nil ACA891 crew turned off fan, smell dissipated and a/c cont'd to Toronto. Maint disabled & deferred the item.
29 Sept. Dayton Ohio Emb RJ145 of US Airways High pressure air-leak stirred up dust in cabin and flight ret'd Dayton. Nil / 50 o/b US 2803 was emerg evac'd after landing.
01 Oct 1455L Liege (Belgium) 747-271 of Cargo Airlines 4X-ICM Flt ex Tel Aviv via Nairobi overran 05R crushing the 23L appch lights. Nil Eng #1 dismounted, badly damaging fuselage. 117 tons of fish aboard.
02 Oct. 1138Z Toronto City Ctr SA226AT of Perimeter Airlines PAG921 veered off r/way 26 on landing, stopping on grass 30ft offset. Nil Nosewheel steering wiring found to be faulty and restricting NWS authority.
02 Oct. 1530L Atlanta, Ga. 737 of Delta Flt: DAL1189 Hole was punched in a/c left wing by truck while parked at Gate A11. Nil A/C taken out of service.
02 Oct. Kuala Lumpur A330 of MAS Flt: MH127 Perth-bound flight cancelled due to wiring sabotage found 2hrs prior t/off. Nil / 300 o/b At least 7 bundles of wire found cut including one behind a pilot's panel.
03 Oct. 1800L Mumbai Intl India A320-200 of Indian Airlines Flt IC107 pilot lost control and bogged after advised of his runway incursion and clearing due other r'way traffic. Nil / 61 pax Main runway closed 12 hrs. Incursions lead to excursions. Incident happened due to clearing runway urgently.
03 Oct 1130L San Bernardino, Calif. (Former Norton AFB) P2V7 Neptune water-bomber of Minden Air Corp N299MA crashed in San Bernardino National Forest 8 miles north of airport on approach over mountainous terrain. 2 fatal / 2 o/b 8th tanker crash in the last decade. A/c arr fm Prescott Ariz seen to enter fog on approach r/w 24 (4mls NE Redlands).
03 Oct. 2130L Waikanae NZ (NE of Wellington) Convair 580 of Airfreight New Zealand A/c disappeared off radar while flying at FL170 from Christchurch to Palmerston North in heavy weather. 2 fatal / 2 o/b Suspect lost flt insts due lightning strike - followed by a break-up in a classic high-speed graveyard spiral.
03 Oct. Sydney Australia 747-400's of QANTAS x 2 (bought 1998) 2 ten y.o. a/c grounded after cracks of 75cms found in VH-OED - near the vertical fin attach-point (FS2181). Nil Both ex MAS a/c undergoing heavy maint checks. (VH-OEC & VH-OED). Sealant removal method suspected.
03 Oct. 2011L Seattle, Wash. MD-80 of Alaskan AK752 A/c taxied into non-movement area and braked suddenly without warning. 1 inj Flight attendant fell and broke her wrist.
04 Oct. 1155L Farmington, NM A320 of America West Flt: 231 Flt ex Phoenix landed 4 Corners Regional A/P with smoke in cockpit. Nil Coffee-maker found to be shorted out.
04 Oct. 1850L Boston, Mass. 737 of American Reg: N972AN While parked at Terminal B Gate 26 a/c was struck by an unoccupied truck. Nil AAL847 Flight pax deplaned & flt cx due skin puncture fwd of #1 engine
05 Oct. 0145L Miami, Fla. A330 of Edelweiss Flt:565 Uncontained RR eng failure in climb (deptd 0128 and returned Miami 0211) Nil / 175 o/b 40nm out in climb turbine bits damaged L wing and penetrated cabin.
05 Oct. 2145L Boston, Mass. 737-700 of Alaska N625AS Alaska Air 15 settled onto a ladder beneath the wing while refueling. Nil Pax deplaned due hole punched in leading-edge slat (as wing dropped).
05 Oct. 0206L Kahualui, Hawaii 757 of UAL Flt: UAL1656 Kona to San Francisco flight diverted due to smoke in cabin. 1 inj / 116 o/b Evacuated via 4 main slides - no further details.
05 Oct. Chicago (O'Hare) 727 of Planet Airways Left main gear caught fire after landing O'Hare Airport. Evacuated. Nil Pax evacuation by emergency chutes.
05 Oct. 2300L Essendon A/P Vic Two hangar doors Two separate arson attacks damaged hangars (one of which cont'd 8 planes) Nil Arsonists disturbed by Security at this secondary Melbourne Aust airport.
06 Oct. 2100L Changi A/P Singapore 747 of SIA Flt: SQ318 Flight delayed by bomb threat hoax due to detailed aircraft inspection. Nil / 127 pax Pax deptd six hrs late. (last such threat was to LA bound SIA flight in Oct 2002).
06 Oct. 1200L Changi A/P Singap 737-400 of MAS Flt: MH601 Runway one closed by flt arriving from KL veering off onto the grass. Nil / 152 o/b Minor damage. Runway opened six hours later.
06 Oct. Charlotte, NC 777-200 OF BA Flt: BA2227 Diverted due smoke in cockpit while enrt London Gatwick to Atlanta. Nil No further details.

07 Oct. 0500L Houston, Texas MD-80 of Unknown Flt 973 a/c taken out of service after forklift left 3-inch hole in fwd fuselage. Nil Hole 3-inch by 3-inch caused by carelessly handled forklift.
08 Oct. 1510L Burbank, Calif. 737-700 of Aloha Flt: 461 Landing runway 8, ingested birds into both engines. Nil Arr fm Las Vegas - damage to right wing, right main gear and nose-gear.
08 Oct. 1510L Phoenix, Ariz. CRJ-200 of Mesa Struck a small piece concrete after ldg and right trailing edge flap damaged Nil Runway FOD (foreign object damage).
