10 Sept. 2310Z Regina Saskatchewan 737-281 of WestJet C-FTWJ ret'd with electrical problems (CB's tripping, #2Gen Off Light) plus other warning lights & elec trim inop Nil Gen replaced (due identical occurrence 06 Sep). Proved to be a fault on #2 Gen Phase B power circuit

18 Sept. 15 NW of Tulsa Oklahoma MD-80 of American Flt 490 took evasive action based on a TCAS alert for 4 F-15 mil jets at FL290 escaping from Hurricane Isabel 5 inj / 90 o/b (3 F/A) Oklahoma City-St Louis. F-15's were some of 70 AF jets repositioning from Seymour Johnson AFB to Tinker AFB
18 Sept. Atlanta, Ga. 747 of SAA Flt: SA7799 53yo US pax tried to storm cockpit, claiming his life was under threat. Nil / 224 pax arrested by FBI on arrival at 0830L (involved in similar 1987 incident)

19 Sept. ca 2000L La Quemada Mex C130A Hercules Mexican Air Force Crashed enrt 2hrs 40 minutes into flt Mexicali to Santa Lucia AirForce Base 6 fatal / 6 o/b Write-off - cause unknown (model dates from early 1960's).
19 Sept. Glasgow DO328 of Tyrol Jet Services VIP Jet OE-HMS wing struck building on taxi-in to parking. Nil Pax included country band "The Dixie Chicks" (minor damage).
19 Sept. 1710L Del Rio, Texas Lear25B cargo of Ameristar Air Cargo N666TW Arr fm El Paso Tx, a/c overshot runway end, crossed major boulevard into cemetery and caught alight. 1 fatal / 1 critical injury >1000ft of skidmarks in last part of runway (landed long and hot & didn't realize until too late?). CVR rec'd.
20 Sept. 1255L Grand Canyon Astar helo of Sundance Helos Sightseeing helo crashed into canyon wall in Meadview -60nm E of L Vegas 7 fatal / 7 o/b Departed Grand Canyon West A/P for the Canyon bottom.
20 Sept. Fukuoka Japan A300 of China Airlines Taipei-bound flight twice returned Fukuoka with nosewheel problems. Nil Recurring maintenance snag.
20 Sept. 1856L Beaumont Tx Hawker 700 of Star Flite Avn Tx N45BP crashed 15nm fm airport after disappearing off radar on trng flight. 3 fatal / 3 o/b Jet seen to spin in after odd engine noises noted by ground observers.
21 Sept. Syracuse NY 767 of United Reg: N615UA NY-LA Flight divtd losing oil pressure. Leaking oil caught fire. Nil / 88 o/b Flt UAL4. ARFF doused fire after pax had deplaned. (Hancock Intl.)
22 Sept. 747 of Saudia Birdstrike flamed out eng on appch. Nil Jeddah-Chennai Intl (Madras)
22 Sept. 2238L Beech E90 44GK Landed gear-up (as rt MLG had failed). Nil Dallas Love Field - major damage.
22 Sept. 1000L Gatwick 737-436 of BA G-DOCT Pisa-bound flight returned/landed with a smoke in cabin emergency. Nil A/c had just had both heat-exchangers replaced.
22 Sept. Gulfport Biloxi Intl 737-700 of Continental COA133 deptd runway side on landing but rolled back onto a taxiway. Nil Minor damage.
23 Sept. 1955L PA-28 & PA-46 N8604N & N146PM collided intxn r/ways 7 & 12R. Both destroyed. 2 inj / 2 o/b North Las Vegas Airport Nevada - PA28 landing, PA-46 on takeoff roll.
23 Sept. 0920L Hyderabad India ATR-42 of Air Deccan After start of 2nd eng (on port side) it caught fire (extinguished as pax evacuated). Nil / 48 o/b Inaugural flight to Vijayawada with India's Aviation Minister aboard.
23 Sept. 0515L Nantucket Mass C402 of Island Airlines Newspaper delivery flt hit embankment on appch in fog 1 fat/1 inj 2 o/b Pre-dawn darkness and fog are the likely factors.
24 Sept. 1120L Washington Nat'l. 737 of United UAL604 Due n/wheel failure a/c slewed to a halt on ldg runway & blocked it for hours. Nil / 57 pax & 5 crew Unaware loss of n/wheel tire on t/off Chicago (right engine damaged on landing).
24 Sept. 1230L Gatwick, UK 767 of Delta A/c vectored for dumping prior to a normal landing 08R with a hydfail. Nil Single system failure on a/c routing from Amsterdam to New York.
24 Sept. 1038L Luxembourg Embraer ERJ-145 Luxair's LX-LGZ Departed r/way 06 rt side on landing and stopped just short of steep incline. Nil / 12 o/b Ex Wien -substantial damage -cause unknown (http://www.iasa.com.au/video.htm).
