21 Dec 1617Z Winnipeg EMB110P1 Bandeirante C-GSKD reqstd return after dep r/way 31 Winnipeg due to smoke in the cockpit. Nil Glycol de-icer entered bleed-air heating system.
21 Dec Toronto-V'couver MD-80 of JetsGo C-GKLJ left engine failed near Medicine Hat. Nil A/C diverted Calgary. Extensive turbine damage found.
22 Dec Varadero Cuba A320-212 of Zoom Flt: OOM743 "Flt controls flap locked" ECAM message on approach. Nil C-GZUM's flaps reset by maint via cycling of wing-tip brake CB
27 Dec Ft McMurray Alb Jetstream 31 Flt: AM32 shut down RH eng for vibration prior to landing. Nil Maintenance found a failed generator bearing.
28 Dec Vancouver A320-212 of SkyService SSV648 reptd ECAM msg of "locked flaps" and landed. Nil Wear in torque link allowed trip of asymmetry sensor.
30 Dec Toronto- Baltimore Md CL600-2B19 Jazz 932 shut down RH eng after exp 1000lbs fuel imbalance & crossflow/APU pump light. Nil Fuel computer and fuel pump replaced.
30 Dec Toronto A319-114 Reg: C-GAQZ Flt. ACA184 dep Kelowna & landed Toronto after hyd fail and manual gear extension. Nil Towed off runway due to nosewheel steering unavailable following manual extension.
30 Dec Montreal F-100 of AA Flt. AAL744 landed 15mins after declaring an eng problem. Nil O ring in oil filter allowed engine oil loss.
01 Jan London (STN)-Eindhoven 737 of Ryanair A/C diverted Rotterdam when info rec'd that Eindhoven had closed for Xmas holidays. Nil No further comment.
01 Jan Toronto A320-211 of Air Canada C-FMEQ (Flt. ACA6081) returned after a compressor stall. Nil Stalled on climb-out. Engine replaced.
02 Jan Herrera A/P Santo Domingo (Dominican Rep) Citation CJ2 Reg: N390RD Overran after fast nosewheel first touchdown and porpoising on landing on 4500ft runway. Nil/4 on board Wings torn off. No fire and fuselage relatively intact. Porpoising upsets lift-dump logic - so no spoilers.
04 Jan Telluride Co HS-125 (700A) of Crusader Avn Reg: N270KA Smoke in cockpit during taxi for t/off. Shutdown plus emergency evacuation. Nil Venturi fan unit rear of aft bulkhead found charred. Insulation, air ducts and wiring found charred.
05 Jan Belfast NI A321-231 of British Midland Within 24hrs two BMI A321's damaged during de- ice ops. Nil Truck hit fuselage G-MIDK on 04.01 and horizontal stabilizer (G-MIDF).
08 Jan 0235L Seattle-Medford DHC-8 of Horizon Air Encountered flock of geese on approach with multiple strikes. F/O took over to land after pilot injured. 1 inj/32 on board Damage to left fwd cockpit section. Pilot suffered multiple cuts and bruises.
09 Jan 0843L Chiclayo-Chachapoyas Peru F-28-1000 of TANS Reg: OB1396 A/C wreckage found near crest of 11,000ft mountain NW of airport - about 10 miles N of landing threshold for runway 31. 42 pax + 4 crew fatal/ 46 on board Low clouds and fog. First hull loss for TANS, 37th Hull loss for F28 /248 built. Total F28 fatalities =769. 12th jet hull loss in Peru. A/C missing 2 days.
09 Jan 0320L Bruss-E. Midlands A300-B4 of DHL Veered off runway during landing in icy conditions. Nil Undercarriage bogged in mud. Minor damage.
Date TBD Guam (Agana) A330 of PAL Snagged high tension wires/trees on Nimitz Hill during GPWS go-round (& then landed Guam). Nil 1ft x1ft hole in hull. Belly damage unnoticed until arrival Manila next day when cargo hold door unable to be opened. Landing gear damaged also.
11 Jan Toronto A320 of SkyService Frozen body of a/c mechanic found in maintenance bay aft of MLG wheel well. 1 fatal Believed boarded in Punta Cana (Dom Rep) five days previously and was well traveled. Foul play not suspected.
12 Jan Concorde of AF Flt: AF002 Nose droop-snoot failed to come up after t/off. Nil/38pax Returned Paris (CDG) as supersonic flight not possible.
12/13 Jan 777 of BA Reg: VIIP Diverted Terceira (Azores) with engine failure (surge). Pax retrieved by two 757 a/c. Nil GE90-85b VSV actuators had feedback LVDT fail and FADEC closed stator vanes, causing rundown.
