19 Sept. 1220L Tokyo, Japan 747-200 of Orient Thai Pilot suspended after passing within 200m of Tokyo Tower on approach. Nil Flying at 540m passed over the 333m high tower on approach Haneda Intl.
08 Oct. Anchorage, Alaska MD-11F of World Awys N275WA sat hard on its tail as power was applied to engs 1&3 (2 eng ferry). Nil / 2 o/b 4-week repair. Crew possibly had forgotten about CG movement when center-gear is retracted.
14 Oct. Orlando, Florida 737 & A321 of US Airways Apparent holes were found punched in the two aircrafts' skins. Nil Another incident on 20 Oct at Charlotte, N.C. FBI investigating.
14 Oct. 0014Z Las Vegas, Nevada Falcon 200 of 489BB LLC A/c became disabled by smoke in the cockpit during taxi and was evac'd. Nil No further details.
15 Oct. 1115L Schiphol Netherlands A330 of Northwest A possible pax bag mismatch forced a return to Schiphol Airport. Nil / 298 o/b Amsterdam-Detroit flight was 3:15 into the flight, returned, and departed after re-check.
15 Oct. Dortmund, Germany A319 of EasyJet 40 dead birds found on runway after a/c turned back after take-off. Nil / 128 pax 5 fanblades and the flight-deck windows were damaged.
15 Oct. 2220Z Enrt NY to HKong A340 of Cathay Flt: CPA831 Declared Mayday for smoke at 2220Z and landed Toronto at 2359Z. Nil Fuel-dumped 2230Z to 2310Z. Initially went for Montreal.
15 Oct. 0746L Medellin, Colombia DC-3C of Aerovanguardia Cargo flt diverted to a local alternate due fog, hit high tension wires, crashed. 3 fatal / 3 o/b HK-1503 was on approach to Enrique Olava Herrera Airport. Destroyed.
15 Oct. 0314Z Vancouver, BC A320-211 of Air Canada ACA139 Declared emergency for hyd failure (no flaps & no nosewheel steering). Nil Ottawa to Vancouver. Towed to ramp after landing.
16 Oct. 1530L Palawan Philippines BN-2 Islander of Pacific Airlines RP-C1325 crashed 30mins after t/off on Mount Tagbao in Barangay Burac. 2 fatal / 2 o/b Crashed in heavy rain 5nm NE of Coron airstrip with its live fish cargo.
16 Oct. morning Chitose, Japan 747 of KLM Flt: KL861 Tokyo Amsterdam flt landed Chitose after eng problems 682mls N of Tokyo. Nil / 284 o/b Landed Chitose (Hokkaido) 45 mins later & pax headed Narita on a spare.
16 Oct. 1800Z Williamsport, Pa. Dash 8 of Allegheny AL03869 landed runway 8 after taking a lightning strike to the vertical fin. Nil Extent of damage unknown.
17 Oct. 1845L Greensboro, N.C. 767-300 of Delta Flt: DL871 Crew decided to divert in after detecting smoke in the cockpit. Nil / 252 pax +7 crew Landed Piedmont Intl during flight from Atlanta, Ga. to LaGuardia, N.Y.
17 Oct. afternoon Mumbai, India 737 of Jet Airways A/c hit by a reversing high-lift catering truck, causing a downing discrepancy. Nil Santacruz A/P.The reason it is high-lift is to avoid the need to reverse into a/c.
18 Oct. 1800L Edinburgh, Scotland 737-300 of EasyJet EZ242 Pilot rejected the take-off when his side window flew open at speed. Nil / 138pax Window latches possibly unsecured.
18 Oct. 2000L Taipei, Taiwan A320-232 of TransAsia GE536 Veered off runway after landing at Sung Shan Airport & nosed into ditch. Nil / 106 o/b Closed the arrival runway 10 - arrival from Southern Taiwan city of Tainan.
18 Oct. 1415L Kerala, India IL-78 of Indian Air Force A/c returned Kerala Intl after 2:45 airborne with a main hydraulic failure. Nil Enroute to Mauritius.
19 Oct. 0035Z Flint, Michigan CRJ-100 of Comair N7441 (COM227) aborted its t/off after hitting two deer on the runway. Nil Deer at twice the price (substantial damage to all involved parties).
19 Oct. 1933L Kirksville, Mo. Jetstream 32EP of Corporate Airlines Crashed 6.5kms S of Municipal airport during non-precision approach in rain. 13 fatal / 15 o/b American Connection flt CEA5966 crashed in a wooded area (N875JX).
20 Oct. Mumbai, India 737 of Alliance Air Flt: CD7130 Flight from Nagpur; emergency landing with hydraulic failure. Nil 55 pax +6 crew Landed Chhatrapati Shivaji Intl and was towed to the ramp.
20 Oct. El Palomar AFB 757 of Argentine AF VIP Sqdn. Argentine president's plane diverted in with a eng fuel control pump failure. Nil 4th aerial event for El Presidente (port engine seen to be spitting flames).
20 Oct. 0407L Cold Bay, Alaska 777-200 of Continental ETOPS Flt CO6 diverted in with an engine precautionary shutdown (oilpr). Nil / 241 pax +15 Tokyo-Houston flt. 10,000ft runway is 642nms SW of Anchorage. 18hr delay.
20 Oct. Santa Rosa Beach, Fla. BO-105S of AirHeart Medical Crashed into bay (positioning for transfer of a critically ill patient to Pensacola) 3 fatal / 3 o/b Metro Aviation operator (pilot, nurse and paramedic). Weather accident
21 Oct. 0758L Cardiff Intl Wales 737 of BMI Baby Flt WW5951 ingested a bird 15 mins after take-off and returned Cardiff. Nil / 127 pax +5 crew Cardiff to Paris flight swapped a/c and departed for Paris at 1406L.
21 Oct. 2345L Detroit, Michigan 747-132F of Kalitta Cargo -former Kittyhawk Reg: N709CK Flight CKS825 [Anchorage to JFK via ORD] diverted in for the longer r/way after making a delivery (dropping off its #1 port outboard 8850 lb wt JT9D-7A motor into Lake Michigan).
O'Hare for JFK. Fuse-pin failures theoretically should not happen after stainless steel non-corroding pins were used.
