15 Nov., night Resistencia, Argentina 737-2S3 of Rutaca YV-216C diverted Resistencia A/P after an engine failed ex Caracas. Nil Pax included Venezuelan soccer team traveling Buenos Aires for World Cup.
18 Nov. 2110L Chicago O'Hare, Ill. CRJ200 of Air Wisconsin, Flt 368 A/c taxiing to Gate F4 at Terminal 2 when a high-speed van hit the rt wing & flipped. 3 in van injured Substantial damage to right wing leading edge (fwd spar undamaged).
19 Nov. Lisbon, Portugal A340-642 of Iberia Reg: EC-IZX Madrid-Mexico flt diverted into Lisbon with smoke in a cargo hold & evac'd. 9inj / 302 o/b Airport closed for 4 hrs. Captain later died of a heart attack.
19 Nov. Shearwater Base Halifax Nova Scotia V22 Osprey (under winterization testing program) Lost a 20" x 4" long piece of rotorblade during de-ice tests ex Shearwater Base and a/c made emergency landing there. Nil Blade heating element likely shorted out creating high localized heating. Centripetal force took care of the rest.

20 Nov. SE Swaziland, Africa Arava 201 of Swazi Defence Force Swazi Defence Force left with three a/c (inc 2 helo) after a/c crashed in bad Wx. Nil / 2 o/b Landed to avoid weather and later took off with control lock still in.
21 Nov. 0820L Baotou Inner Mongolia CRJ200 of China Eastern Shanghai bound flt MU5210 crashed into an icy lake immediately after take-off. 54 dead / 53 +1 on grnd Suspect failure to de-ice plus wing ice ingestion led to an aerodynamic stall.
21 Nov. 1738L Denver, Colorado MD82 of AA Flt: AAL1115 N234AA landed 1,000ft short of r'way 35L threshold (in freezing fog, low overcast) Nil / 108 o/b A/c engines, airframe and tires damaged, approach lighting damaged.
21 Nov., night Bangor, Maine B777 of Air France Flt: AF026 Paris-Washington flt diverted when 2 Moroccan pax found to be on "no fly" list Nil / 263 o/b Unairworthy citizens deplaned and flt arrived two hours late at Dulles.
22 Nov. 1642L Glasgow, Scotland EMB145 of BA Southampton bound flight BA7856 returned with smoke in the cockpit. Nil / 50 o/b Took off 1610L & returned 1642L
22 Nov. 0630L Gardermoen, Norway SAS MD80 & B737 in heavy snow Copenhagen bound SK1457 cleared line up in front of SK336 arriving fm Bergen. Nil / 200 o/b Taxiing a/c captain refused his line-up clearance. Oslo ATCO suspended.
22 Nov., afternoon Pontianak Indonesia B737-500 of Garuda Flt: GA501 Captain collapsed and died of a heart attack 10 mins after dept. A/c returned. 1 dead / 107 pax / 7crew Pontianak Borneo for Jakarta. Capt had complained of ill health earlier in day
22 Nov 1214Z Hobby A/P Houston, Texas G-1159A of Biz Jet Svcs, Reg: N85VT 600ft below glidepath, clipped 120ft light pole on finals rwy 4 and crashed onto cars. 3 dead / 3 o/b A/c positioning to take Pres G. Bush Sr to Ecuador. Scud Running in fog?
23 Nov. 1100L Goa, India A300 of Indian Airlines IC598 had an eng failure shortly after departing for Mumbai and returned. Nil / 250 o/b 06 Nov an IA A300 hit a tractor & then dept'd Mumbai without inspect.
25 Nov. 0305L Rockhampton Qld Metroliner of PalAir Express Mail-carrier cargo a/c badly damaged when left main gear folded upon landing. 2 uninj / 2 o/b Damage to wing, prop, engine, aileron and pride.
25 Nov. 2355L Evenes, Norway A320 of MyTravel G-CRPH (bound Norwich UK) lost t/off directional control & snow-plowed off. Nil / 123 o/b Became snowbound in deep snow. A/P closed for 5 hrs while a/c retrieved.
26 Nov. Boston Logan A/P Massachusetts 3 candidates for the "iceman cometh" award Family noted a 2ft x 4ft hole in ceiling & FAA narrowed down source of basketball sized ice-chunk found on floor to three arrivals within that time-zone Nil (Meanwhile an Auckland NZ identical event reported a rugby ball size ice). Security now seeking globe-trotting sporting teams with drink problems.
27 Nov. Bamiyan, Afghanistan CASA 212 of US Mil Charter Contract Civilian a/c departed Bagram A/P and disappeared in Central Mountains. 6 dead / 6 o/b (3 civ crew) Debris discovered atop snow-covered 16,600ft Baba Mountain (bodies recovered).
28 Nov. 1850L Barcelona, Spain 737-400 of KLM Reg: PH-BTC KL1673 skidded off runway 25R during landing at El Prat A/P & gear collapsed 10 inj /139 pax +6 crew Undercarriage seriously damaged. Other a/c movements delayed 2hrs.
28 Nov. 0955L Montrose, Colo. Challenger 601 of Jet Alliance GLB73 N873G crashed on take-off runway 31 after a 40 minute opstop and refueling. 3 dead / 6 o/b Failure to de-ice plus 1/4 inch rain & slush on runway. Vis 1/4 mile in snow.
29 Nov. 1937Z Vail, Colo. Gulfstream IV of Netjets N420QS veered off the Eagle runway to right on landing & nose-gear collapsed. Nil / 3 o/b Deptd paved surface about 1,000ft in and blocked runway - 3 flts diverted.
30 Nov. 0610Z Philadelphia, Pa. MU-2 of Epps Aviation EPS108 Fuel spill after turboprop's wing clipped a tug during take-off on the N-S runway. Nil Tug was towing Delta a/c to Terminal E. ATC grnd controller is suspended.
30 Nov. 0156Z Howell Island, Missouri HFB320 HansaJet of Grande Air Express N604GA crashed shortly after t/off St. Louis, Mo., in low cloud & icing conditions. 2 dead / 2 o/b Headed Toledo Express Ohio. Engine failure. No CVR & no DFDR aboard.
01 Dec. 2123Z Teterboro, N.J. Gulfstream GIV Reg: G-GMAC Ex reg'd VP-BME went off rt at end into trees while landing in blustery conditions Nil / 7 o/b Right wing sheared off and nose-gear collapsed on arrival from London.
01 Dec. 1815L Solo, Indonesia MD82 of Lion Air Flt: JT583 PK-LMN landed > 1/2 way in on very wet runway 26 & skidded off into a cemetery 25 dead / 61inj / 153 o/b Adi Sumarmo A/P (310 NM SE of Jakarta). Suspect spoilers not armed.
02 Dec. 1857L Bangkok Thailand 777-300 of Cathay Pacific Flt: CX751 Inner surface of #1 eng LH duct collapsed after t/off - causing separation & detachment of a 1.5m x 2m section of the duct structure (which fell to ground). Nil/345pax + 17 crew Left side duct of thrust reverser housing severely damaged a car below. 3rd CX engine event since 10 Nov. Component belongs to airframe (& not engine).
