August 01, 2009 Nairobi, Kenya CESSNA U206 N5Y-LMB A/c owned and operated by African Inland Mission collided with the ground. 1 fatality, one serious A/c conducting low flight while passengers filming underlying area. During accident sequence, a/c struck power line and building. Pilot fatally injured.
August 01, 2009 Deer Island, ME EUROCOPTER EC130 M-BOAT A/c substantially damaged during forced landing on the water near Deer Island. Flight departed a privately owned motor yacht. Four minor Flight flown to coastline at 400 feet above mean sea level. Pilot began maneuvering a/c which began loosing altitude. Prior to impacting water, pilot deployed emergency flotation devices mounted on skids to prevent a/c from sinking.
August 01, 2009 Caldwell, ID Kenneth Gray RV-6 N69KG A/c destroyed following impact with terrain near Caldwell Industrial Airport (EUL). One fatality Witness observed a/c depart to NW. Heard engine "sputtering." Pilot was turning back toward airport when a/c collided with set of power lines,
Aug. 02, 2009 Santa Monica, CA Davenport DAVE-EZ N7CE Experimental amateur built a/c sustained substantial damage following uncontrolled descent and impact with terrain at Santa Monica Municipal Airport (SMO). One serious Pilot said taking a/c up to check the engine. Pilot requested "box pattern" after takeoff. After takeoff, pilot reported control problems; A/c observed "wobbling" back and forth before descending in what appeared to be an uncontrolled descent.
Aug. 03, 2009 Torrance, Canada Cessna TU-206 C-GGMG A/c impacted trees during takeoff from Lake Muskoka. Two fatalities A/c destroyed. Pilot and passenger sustained fatal injuries. Flight was originating at time of accident.
August 03, 2009 Bavaro, Dominican Republic BOEING 767-224 N76156 Continental Airlines Flight 128 enroute from Rio de Janeiro, Brazil (GIG) to Houston Intercontinental Airport (IAH), Houston, TX, encountered turbulence at an altitude of 36,000 feet. 33 minor Flight crew declared emergency and diverted to Miami International Airport (MIA). 28 people assisted by paramedics and 14 people transported to hospital; 13 treated and released and one person was admitted and released following day. Five flight attendants were treated and released upon arrival in Houston.
August 04, 2009 Louisville, KY BOMBARDIER CL-600-2B19 N8543F Pinnacle Airlines Flight 2871 enroute from Knoxville, TN to Detroit, MI when flight encountered turbulence. Flight had been operating at FL300 and had just initiated a climb over cumulonimbus clouds when turbulence was encountered. One serious One flight attendant received a serious injury and one passenger received a minor injury. Flight attendant was transported to local hospital and passenger was treated at airport and released.
August 04, 2009 Happy Jack, AZ BOEING E75 N5195N A/c substantially damaged when impacted terrain while maneuvering. One fatality, one serious Passenger said after departure from Cottonwood, a/c had difficulty gaining altitude during cruise flight. Pilot decided to turn back towards Cottonwood. Passenger looking at sectional map attempting to verify terrain elevations in area of intended flight path when he suddenly blacked out.
August 07, 2009 Gladwin, MI HOOPER JAMES A BUSHBY MUS N37JD A/c destroyed when impacted trees and terrain while maneuvering. Radar data show a/c departed 3BS from runway 06 and made turn to N. A/c made a series of 3 turns between 2,500 and 2,700 feet msl. A/c then made a turn back toward 3BS and shortly thereafter radar contact was lost. One fatality On the morning of August 8, 2009, the A/c reported missing by family members. A search initiated and a/c located in wooded terrain. Examination of accident site revealed a/c impacted terrain in nose low attitude.
August 07, 2009 Saint Charles, ID PIPER PA-22-150 N6905B A/c impacted terrain. Witnesses reported a/c flying over their position at about 300 feet above trees on a northerly heading. Two fatalities Examination of accident site revealed that a/c impacted open field within a heavily wooded mountainous area.
August 08, 2009 Calhoun, TN Stump Bradley Kolb N2456 A/C substantially damaged following loss of control in flight. Witness observed a/c climb straight up and then do a "cartwheel" and come straight down then level out. Two fatalities A/c did this maneuver three times; however, on third time, a/c went straight up and "flipped over backwards, started spinning, and collided with the ground".
August 09, 2009 Bumpass, VA MAXAIR Drifter N160HA A/c substantially damaged following loss of control after takeoff from surface of Lake Anna. Witnesses said "appeared to stall" before striking water. Passenger egressed a/c under own power, but several minutes elapsed before pilot could be freed from wreckage. One fatality One witness described application of full power. Witnesses described steep left bank almost immediately after takeoff before a/c descended nose down into the water and submerged.
August 10, 2009 Tokyo, Japan BOEING 737 JA56AN Air Nippon Flight ANA298 took off from Tottori Airport and landed at runway B of Tokyo International Airport. No injuries Post flight check showed that lower tailpiece of fuselage damaged about five meters long and one meter wide. Two scratch marks (about 25 meters long and 7.5 meters long) were made on runway B of the Tokyo International Airport.
August 19, 2009 Mackay, ID Stemme GMBH & CO S10-VT N35MP A/c collided with mountainous terrain. Pilot departed with two other gliders, with intention of soaring in area of White Knob Summit. Two other pilots were in radio contact with accident pilot until about 1630, at which time he said soaring in area of Mackay reservoir. Pilots attempted to make radio contact about 45 minutes later, but unable to contact accident pilot. One fatality Local law enforcement & CAP initiated search. Glider located following afternoon on western face of White Knob Summit, at elevation of about 9,700 feet msl.
August 22, 2009 Ras Tanajib, Saudi Arabia AGUSTA A109E, N906AH Helicopter operated by Saudi Aramco Aviation sustained substantial damage during landing at barge Arabyia 4, in Persian Gulf. No injuries Pilot and 4 passengers not injured. Visual meteorological conditions prevailed for flight that departed Ras Tanajib Airport (OETN).
August 23, 2009 Creswell, OR PIPER PA-25 N6353Z Piper tow airplane collided with terrain about 1/2 - mile N of departure end of runway 33 at Hobby Field Airport. One fatality Witnesses said a/c departed Runway 33 with Schweizer glider in tow. Shortly after takeoff tow plane impacted terrain and burst into flames. Glider returned to airport and landed without further incident.
August 23, 2009 Santa Barbara, CA PIPER PA24-260, N8775P A/c collided with automobiles during emergency landing on a freeway. Five miles from Santa Barbara, engine began to run rough and lose power. No injuries Pilot turned the fuel boost pump on, and switched back to left main tank. Unable to restore full engine power, pilot declared emergency and prepared to make off airport forced landing.
August 24, 2009 Arcanum, OH DESTINY 2000 powered parachute N5604B Powered parachute and unregistered Phantom ultra light collided in-flight at private airstrip near Arcanum. One fatality, one serious Witness reported that a/c collided over W end of private airstrip. Powered parachute on final approach to land to the E, as Phantom ultralight approached from S. After collision, powered parachute descended onto runway. Ultralight impacted adjacent corn field.

August 25, 2009 Rockaway, NJ CESSNA 172R N6117R A/c crashed in mall parking lot during forced landing. Engine came "to a halt with the propeller stopping" with no prior warning of imminent failure. Two minor A/c position one mile E of Rockaway Towne Center Mall. Instructor elected to land a/c in empty section of mall's parking lot.
August 27, 2009 Pavo, GA SOCATA TB20 N654GT A/c encountered adverse weather and crashed into trees and the ground. Fight originated at Montgomery Regional Airport (MGM), Montgomery, AL on same day. Two fatalities A/c reported missing by family members on evening of August 27, and found by local authorities on August 28.
August 27, 2009 Inverness, FL BOEING 717-200 N994AT Airtran Airways Flight 163 encountered turbulence while in a climb. One flight attendant sustained serious injuries One serious As a/c passed through flight level 250, encountered turbulence for 10 seconds. Non-flying pilot checked airborne radar and changed the range settings to see if a/c had "hit a cell." He saw "nothing" on radar.
August 27, 2009 Orland, CA James B. Taplin RV-6 N43521 A/c substantially damage after impacting terrain following loss of control at Orland Haigh Airport (O37). Flight departed Chico Airport (CIC), Chico, CA. Two fatalities Witness heard a loud 'slapping' explosion behind him to the north. Observed cloud of dust in gravel area between runway and taxiway. Did not hear engine/motor sounds.
August 27, 2009 Grand Canyon, AZ AEROSPATIALE AS350 B2 E N705LV Ecureuil chopper impacted boulder during takeoff from Ramada helipad, about 2.2 miles E of Grand Canyon West Airport (1G4), Peach Springs, AZ. No injuries Seconds after becoming airborne as maneuvering during left turn, helicopter's tail rotor impacted boulder. Helicopter began severely vibrating, and pilot immediately landed. A/c remained upright.
August 29, 2009 Brownsville, OR SIX-CHUTER AMERICAN SPIRIT N9184F Powered parachute collided with residential power line and crashed on short final approach to open field. Pilot flew low final approach path and was proceeding in an easterly direction toward the sun. One serious Pilot's powered parachute collided with the top power line, which was not marked. The impact site was about 50 feet AGL. After the collision, pilot lost control of powered parachute.
August 30, 2009 Canyonville, OR PIPER PA-22-150 N2967P A/c iimpacted mountainside about 1.2 miles S of Canyonville. Two fatalities Several witnesses reported hearing sputtering engine, while others reported that engine revved up or down, or was silent seconds before heard boom noise of impact.
Sept. 02, 2009 Kurnoon, India BELL 430 VT-APG Government of Andhra Pradesh Gad Bell 430 crashed enroute to Chitoor, India from Hyderabad, India. Five fatalities There were 2 crew members and 3 passengers, including the Secretariat of Hyderabad on board. A/c destroyed and no survivors.
Sept. 03, 2009 Torrance, CA ROBINSON R44 N8361X A/c substantially damaged during practice autorotation at Zamperini Field Airport (TOA). Pilot initiated practice auto rotation to northern helicopter pad from altitude of 800 feet MSL. As a/c descended through about 100 feet AGL, initiated recovery by slowly applying power. No injuries After applying power, observed engine and rotor rpm increasing toward red line and pilot attempted to regain control of rotor rpm as a/c continued to descend. Due to low altitude, decided to continue auto rotation to ground.
Sept. 4, 2009 Afghanistan USAF MQ-1 Predator UAV Air Force MQ-1 Predator unmanned aircraft crashed in eastern Afghanistan. No injuries Crash not due to hostile fire. No reports of civilian injuries or damage to civilian property at the site.
Sept. 05, 2009 Tulsa, OK PIPER PA-32R-300 N1228H A/c struck radio tower guide wire and subsequently struck ground 8 miles NW of Richard Lloyd Jones, Jr./Riverside Airport (KRVS). Accident site located within confines of Chandler Park, located at 6500 West 21st Street, west of Tulsa, Oklahoma. Five fatalities Accident site began at 600-foot tall radio tower located near center of park. A 7-strand steel guy-wire supporting tower on W side and located 150 feet up tower's base, severed 50 feet out from where wire attached to tower. 300-foot section of wire located with main wreckage 1,575 feet W of tower.
Sept. 7, 2009 Borneo Nomad Indonesian Navy Indonesian Navy a/c crashed into swamp on Indonesian half of Borneo island. Four civilian passengers killed and five others injured when a/c crashed 24 miles W of East Kalimantan province's Sekatak river mouth. Four fatalities Nomad carrying three military personnel and six civilians on a maritime patrol mission. Local residents said plane crashed into cluster of prawn ponds at edge of river. Witness heard sound of roaring engine.
Sept. 13, 2009 Hebron, Israel Israeli AF F-16 fighter Son of Israeli astronaut who died in space shuttle Columbia disaster killed when his F-16 crashed on routine training flight. Onr fatality Capt. Asaf Ramon was son of Ilan Ramon, Israel's first and only astronaut. His fighter crashed S of West Bank city of Hebron
Sept. 15, 2009 Stuttgart, Germany Fokker 100 Contact air a/c forced to make emergency landing on protective layer of foam after landing gear failed to open. Partially open landing gear scrapped along Stuttgart Airport runway All 73 passengers and crew escaped via emergency slides. One passenger lightly injured and flight attendant taken to hospital for observation.
