19 Sept. Memphis, Tenn. MD-11 of FedEx Nasty tailstrike on landing runway 09 during Cat IIIb go-round from flare. Nil Manual throttle-up/no TOGA-press and autopilot suddenly disengaged.
20 Sept. 1200Z Toronto, Canada 767-233 of Air Canada C-GAUB Seated pax noted smoke outside. It was tracked to the avionics compartment. Nil Flt ACA1145 pax deplaned. Charging unit for APU battery was in fail mode.
21 Sept. 1650Z Winnipeg, Canada Cessna 560 Reg: C-GBNX Left leading edge punctured after a/c struck 3 Canada geese after t/off on 31. Nil $600K & 6 week grounding to repair holed fuel cell. Hit geese at 2,000 ft.
21 Sept. 2130L Washington, DC 737-524 of Continental N14654 struck Left engine on jetway whilst being marshaled to gate B23 Nil / 54pax & 5 crew Washington Dulles A/P. Minor damage. Headed in direction of Chantilly, Va.
21 Sept. 1210L Rifle, Colorado SA-226TC Merlin 4 LYM1180 veered off the runway on landing and became bogged in mud. Nil Grand Junction, Colo., for Rifle, Colo.
22 Sept. Salt Lake City 737 of Delta F/O suffered retinal burns from a ground-based laser directed at his a/c. 1 inj Dallas-SLC. FAA states that hundreds of similar incidents have occurred to date.
22 Sept. 0140L North Las Vegas Fairchild F27F N366SB Struck hangar with left wingtip while taxiing out for take-off. Nil / 2 o/b Substantial damage.
22 Sept. 0440L Garden City, Kan. Beech 1900D of America West AMW4698 deptd runway 35 left side on landing in rain, mist & low vis. Nil / 9 o/b Minor damage to N178YV after loss of directional control.
23 Sept. 1100L Kodiak, Alaska BN-2A of Homer Air N6522T Hit wind-shear on beach departure from wilderness lodge and crashed. 1ser / 2 minor / 7 o/b Came to rest on tidal flats in Hallo Bay. Wing torn off, fuselage torn open.
23 Sept. 1450L Kuopio E Finland MD-11 of Finnair Reg: OH-LGB Diverted due to smoke in the cabin an hour before arrival at Helsinki. Nil / 247 pax & 14 crew Osaka-Helsinki flight. Broken bearing in aircon fan. Departed 3.5 hrs later.
24 Sept. 0445L Gwinne, N.D. C208B Caravan N7392B Aircraft crashed on approach 1.5nm from regional airport (no emergency). 1 inj / 1 o/b Fergus Falls, Minn., to Gwinne, N. D. (freight operation).
24 Sept. afternoon Kauai, Hawaii Bell 206B of Bali Hai Helos Tourist helo crashed on remote 3500ft island ridge. 3rd fatal crash in 3 years. 5 fatal / 5 o/b Hazard is instant cloud formation due to venturi pressure drops thru gorges.
24 Sept. 2304Z Toronto, Canada CL600-2B19(RJ) Pinnacle Airlines Reqstd emerg diversion with electrical problems but switched to Buffalo, N.Y. Nil Detroit to Halifax flight then bypassed Buffalo & landed back in Detroit.
25 Sept. 2018Z Fullerton, Calif. BushMaster Reg: N750RW Stalled and crashed in parking lot at airshow after uncoordinated take-off. 2serious / 2 slt inj One of 2 Ford TriMotor replicas. Possibility of flt control malfunction.
26 Sept. 1529Z Stansted, UK A340 of Olympic Flt: OA411 Diverted into Stansted after a specific threat was rec'd by Athens newspaper. Nil / 290 pax & 12 crew Athens-NY. This fight, OA411, divt'd Shannon two days later, 28 Sept. with same threat scenario.
26 Sept. 0800L Anchorage, Alaska MD82 of DoJ (US Marshals) Crew aborted t/off runway 32 after power loss #1 and swerve at >90kts. Nil / 131 o/b Damage to first-stage disk due o'night snow & no intake plugs fitted. N16894
27 Sept. Malpensa Milan, Italy MD-80 of SAS 767 of Air Lauda SAS aborted its take-off at slow speed due Air Lauda a/c crossing runway 35. Nil Air Lauda cleared to cross runway in error. ATC controller suspended.
27 Sept. 1255L Sheremetyevo, Russia 737 of JAT Flt: 132 Yugoslav airliner ran off the runway after landing in mist and rain. Nil / 75 pax & 8 crew Belgrade to Moscow. Pax evacuated and delivered by bus to Terminal 2.
27 Sept. 0330L Las Vegas, Nev. A320 of Ryan International Ryan Intl 9 pilots felt bump on landing and discovered damage to #1 engine. Nil N941LF n/wheel left tire shed tread into #1 engine, damaging fan blades.
27 Sept. 0730 Toronto Canada 767-300 of Air Canada C-GHLA pushed pack by tug from midfield terminal into the blast fence. Nil A/c grounded with tears to the a/c skin around the APU area.
28 Sept. 1540L Ronaldsway, Isle of Man ATR42 of EuroManx Fire broke out in an engine after shutdown on stand (arr fm Liverpool). Nil Flames were extinguished by ARFF and visible damage was minimal.
28 Sept. 1614L Wilmington, Del. C130's x 3 of USAF &Air National Guard 1 x C130 destroyed & two interlocked (after tornado touched down). Three persons injured, much airport damage.
5 mil a/c damaged in total (one C130 flipped & wingless @ New Castle A/P). 15K gallons fuel spilled.
29 Sept. 1050L Bodoe, Norway (530nm N of Oslo) Do-228 of Kato Air Deranged 34yo male pax attacked pilots and pax with his own axe on descent. Do-228 has no cockpit door. 4 inj / 2 serious / 11 o/b Pilots regained control after spiral/spin entry. Fm Narvik (no metal detectors). Algerian asylum seeker reject arrested.
30 Sept. afternoon East Midlands A/P A300 of TNT Cargo Declared an emergency with flaps and instrumentation before landing. Nil / 3 o/b Belfast to Castle Donnington (East Midlands A/P).
30 Sept. 1300Z Schiphol, Holland A319 of BA Flt: BA983 Berlin-London flight divt'd in after a specific security threat was received. Nil Dutch F-16 fighter escort.
30 Sept. 1655L Manchester, UK 777-200 of BA Flt: BAW219 BAW219 declared Mayday diverting Manchester with a fire on board. Nil G-YMMD departing for Denver, Colo. Fire in cockpit area.
