Cross-country flight originated at Memorial Field Airport (HOT), Hot Springs, AR and was terminating at Drake Field Airport (FYV). | Four fatalities | Witnesses heard engine running prior to impact and two witnesses observed a/c shortly before impact in nose low attitude. A/c heard to "circle" over two of witness's location and pass over their home twice within less than one minute before it impacted. Preliminary radar data indicated that a/c varied altitude and heading outside (south of) final approach fix and impacted terrain 11 miles S of approach end of R34. |

Further reduced rpm, and shortly thereafter engine lost power. Thought carburetor ice might be responsible, and immediately added carburetor heat, full throttle, and full rich mixture. He said some power was restored, but engine lost all power. |
| Feb 11, 2010 Cheverly, MD | EUROCOPTER As 365 N2 N61MD | Chopper operated by Maryland State Police (MSP), substantially damaged during takeoff attempt from helipad at Prince George's General Hospital (1MD4). While enroute to 1MD4, pilot advised by hospital personnel to land on ground-level helipad, due to presence of ice on hospital's elevated helipad. Decided a straight-up takeoff with no yaw movement would be made. Slow deliberate takeoff was then initiated, and when a/c "became light on the wheels, but prior to takeoff, a vibration was detected, emanating from rear of aircraft." | No injuries | In response to vibration, pilot lowered collective, shut down both engines, and applied rotor brake. Inspection revealed that " fenestron was severely damaged by tail rotor blades, right tail rotor gearbox cap appeared to have been ingested by tail rotor, and numerous tail rotor blades were damaged." Preliminary exam by MSP and FAA personnel revealed substantial damage to tail rotor blades, the fenestron, the tail rotor gear box (TGB), the TGB drive coupling, and TGB mounting structure. |

At the time, a/c over Santa Clarita, about 20 miles NW of Burbank. | One minor, one serious among flight attendants | Captain advised of approaching traffic during communications with air traffic controller. Initially, the flight crew did not observe traffic. Subsequently, as descending a/c approaching 6,000 feet MSL, crew received traffic collision avoidance system (TCAS) resolution advisory (RA). Captain responded to RA by initiating 1,500- to 2,000-foot-per-minute rate of descent. Thereafter, crew initiated similar rate of climb. |

Witnesses on ground observed a/c fly over R32, with landing gear retracted, at altitude of 50 feet AGL and at much higher rate of speed than normally seen for a/c that low. | Five fatalities | A/c was one third down runway when section of a/c was seen to separate as nose of a/c pitched up. A/c rolled to right and veered toward right, away from runway. A/c impacted ground and continued sliding and rolled as debris departed from a/c. |
