Local mosquito control flight originated at Brazoria County Airport (LBX), Angleton, TX. No injuries Pilot said was spraying a field and made right turn in easterly heading when a/c "hit something." Controls didn't feel right so pilot landed in nearest field. During landing, a/c struck cow and bull, then collided with pile of wood. Investigation revealed a/c collided with radio tower at Clemens Correctional Facility,
Oct 03, 2009 Narrogin, Australia, Australia AIR TRACTOR AT-502 VH-ODP A/c operated by Agricair Pty Ltd. as ag flight collided with ground while maneuvering 47 km E-NE of Narrogin. One fatality Commercial pilot, sole occupant, killed. Flight originated from Wikepin, Australia, at unknown time.
Oct 05, 2009 Albuquerque, NM AEROSTAR INTERNATIONAL RX-7 N9126K During morning balloon competition pilot attempting to maneuver balloon to drop marker on target. When approaching target area flying at 15 feet agl when "burned to ascend" over steel frame sponsor's tent on edge of competition target area. Balloon cleared rear of tent but basket struck front edge. Two serious Review of video shows that impact abruptly tilted basket more than 45 degrees. Passenger was ejected and fell approximately 30 feet to ground. Suddenly minus weight of missing passenger balloon ascended rapidly and pilot attempted to correct by venting. Rapid climb and venting at top of balloon envelope caused mouth of balloon to close and balloon then began descending rapidly.
Oct 08, 2009 Albuquerque, NM AEROSTAR INTERNATIONAL RX8 N4014J Pilot aware that "fuel was an issue" and knew did not have enough fuel to cross congested downtown metropolitan area ahead. Picked a parking lot with one end empty of vehicles as landing area. After crossing wires on edge of parking lot "ripped out pulling vent line as far as pilot could". One serious Pilot joined passengers at bottom of balloon's basket for expected hard landing as balloon skidded across parking lot and struck concrete barrier. Becoming airborne again, basket then struck and damaged building and fell approximately six feet to ground.
Oct 08, 2009 San Diego, CA CESSNA 550 N6776T A/c substantially damaged after impacting object during takeoff roll at North Island Naval Air Station (NZY), A/c registered to US Customs Service and operated by U.S. Customs and Border Protection, Office of Air and Marine. A/c commenced takeoff roll, during which second pilot informed first pilot that runway distance remaining sign was in takeoff path, and needed to veer left to avoid hitting sign. No injuries Pilot made a correction to left, but not soon enough to avoid impacting sign with outboard section of right wing. A/c continued takeoff, departure, and en route climb towards destination. Flight crew assessed damage to the right wing and elected to return to NZY. Flight subsequently made uneventful descent and landing at NZY.
Oct 10, 2009 Alexandria, LA CESSNA 150F N8642S Two Cessna 150Fs substantially damaged following in-flight collision. Both a/c departed Esler Airport (ESF), Alexandria, LA at an unknown time as a two-ship formation. During climb out to enter downwind pattern, lead radioed his intention to turn right. Wingman was concerned about his relative position to lead and radioed that lead should not turn too hard to right. Wingman observed lead airplane bank 45 degrees to right. Oct 15, 2009 La Belle, FL ROBINSON R44 N3038W A/c substantially damaged during forced landing. Flight was a personal sightseeing flight with a friend, and two acquaintances of that friend. While in cruise flight at 60 knots and 150 feet agl, pilot heard a "light tapping" sound and then "a horn went off." One serious Pilot not observe any other problems or annunciations with a/c. Chopper began to lose altitude, and pilot "immediately proceeded into auto-rotation toward pasture." Tail of a/c struck trees.
Oct 15, 2009 Kaunakakai, HI CESSNA 208B N861MA A/c sustained substantial damage when tail impacted ground when ramp agent opened passenger door and boarded a/c at Molokai Airport. Second-in- command (SIC) noted error in load manifest. No injuries SIC communicated error to ramp agent who consequently opened passenger door and entered a/c.. Concurrent with ramp agent entering a/c , pilot-in- command (PIC) exited a/c and, according to representative from operator, a/c "tilted downward" and tail tie-down assembly struck tarmac.
Oct 21, 2009 Summerville, SC PIPER PA-23-250 N62635 A/c destroyed when impacted trees and terrain immediately after takeoff from Summerville Airport (DYB). One witness heard a/c's engines start up, and observed a/c during takeoff roll from runway 24. However, due to location of trees on his property, witness unable to see a/c during initial climb phase. Four fatalities Another witness reported that heard a/c when "in the place where the airplanes sit before they take off," and that engine sounds changed pitch twice. Most witnesses reported noticing that sound of engines was "loud." Some witnesses reported that runway lights not illuminated prior to accident. Several witnesses reported three or four post-accident explosions.
Oct 21, 2009 Hernando, MS Argence Enas A. Christen Eagle N24EA A/c crashed off end of runway 14, at Hernando Village Airpark, Inc., (H75). One serious Witnesses said shortly after takeoff there was a "loud bang or pop" from a/c . Pilot attempting to return to airport when collided with ground in nose down attitude and slid about 70 feet before coming to rest.
Oct 21, 2009 Provo, UT CESSNA T210G N6869R A/c experienced total loss of engine power during southbound climb following departure from Provo Municipal Airport. Pilot reversed course and attempted to return to airport. A/c impacted upsloping terrain adjacent to airport perimeter road while on short final approach to runway 36. One fatality On-duty Provo controller said a/c was few miles S of airport, and pilot stated had lost all engine power. Controller immediately cleared pilot to land. Controller watched the a/c's approach to airport, and nothing unusual noted regarding descent. No unusual maneuvering observed, and no evidence of smoke trailing from a/c.
Oct 22, 2009 Elizabethton, TN CESSNA U206G N120MS A/c registered to Focus Aviation collided with trees while maneuvering at low altitude in Roan Mountains There were no witnesses to the accident. Two fatalities Two pilots on board, certificated commercial pilot who was receiving missionary flight training, and certificated airline transport pilot with flight instructor certificate, were killed.
Oct 23, 2009 Adrian, MI Commander Aircraft 114-B N6025U A/c piloted by an instrument-rated private pilot destroyed during post-impact fire following loss of control while on instrument approach to Lenawee County Airport (KADG). Two fatalities Witness heard a/c over fly residence at low altitude, but unable to see a/c due to heavy fog in area. Noted that a/c had come from N and was traveling to SE. Heard a/c turn N for short time before seeing it pass over residence 200-300 feet agl traveling to SE. Witness noted that a/c's left wing had separated from fuselage.
Octo 23, 2009 Zephyrhills, FL PIPER PA 46-350P N98ZZ A/c substantially damaged when collided with wooded terrain, during emergency approach to Zephyrhills Municipal Airport (ZPH). Three fatalities Pilot declared emergency and requested assistance to nearest airport. Controller provided vector and distance to nearest suitable airport, ZPH, and pilot subsequently reported "engine out, engine out." A/c impacted wooded terrain 4 miles NE of runway 22 at ZPH.
Oct 25, 2009 Bennington, VT CESSNA 172P N55479 A/c substantially damaged following impact with trees and terrain near William H. Morse State Airport (DDH), A/c owned and operated by Sky Training, West Milford, NJ. Witnesses said a/c was in traffic pattern at DDH, and pilot was performing right-hand patterns for runway 13. One fatality Published pattern for runway 13 is left-hand traffic and right-hand traffic for runway 31, due to high terrain S of airport. Witnesses observed a/c attempt two approaches, each followed by go-around about 20 feet AGL. On final pattern, a/c observed on downwind leg, at low altitude, and in level flight. Witnesses stated engine running normally, followed by sound of impact, then silence.
Oct 27, 2009 Kalamazoo, MI BEECH A36TC C-FFIH A/c destroyed by impact forces and post-impact fire following takeoff from Kalamazoo/Battle Creek International Airport. One fatality A/c piloted by private pilot and was on instrument flight rules (IFR) flight plan. Pilot fatally injured. Flight originating at time of accident and intended destination was Muskoka Airport, Muskoka, Ontario, Canada.
Oct 27, 2009 Gaylord, MI AERONCA 7AC N2239E A/c destroyed during in-flight collision with terrain near Gaylord.. Two fatalities Pilot and passenger sustained fatal injuries. Flight departed Gaylord Regional Airport (GLR).
Oct 29, 2009 Loreto, Peru AEROSPATIALE AS350 OB-1909-P A/c operated by Helinka, had total loss of engine power while descending to remote landing zone. Two fatalities, One serious Chopper collided with trees during forced landing. A/c received substantial damage. Commercial pilot and one passenger killed. One passenger received serious injury. Flight originated from remote landing site at Loreto.
Oct. 29, 2009 Amazon, Brazil C-98 Caravan Brazilian military transport was flying from Cruzeiro do Sul, in northern Acre state, to Tabatinga in Amazonas when it issued emergency distress call 58 minutes after takeoff and then lost radio contact. 11 missing Plane carrying 11 people -- four crew and seven passengers -- and was supporting Ministry of Health vaccination mission.

Nov. 2, 2009 Mirny, Russia Ilyushin IL-76 All eleven people on board this Russian cargo plane were killed when the aircraft went down shortly after takeoff. 11 fatalities The Ilyushin IL-76, owned by the interior ministry, crashed near the eastern city of Mirny. An interior ministry press service official says the plane rolled to the right immediately after leaving the ground. He said it flew on at a height of no more than 30 meters (98 feet) before crashing near an old mine up to two kilometers (1.2 miles) away.
