November 3, 2011 Key West, FL Cessna 550 N938D Around 1212 EDT, a/c received minor damage during a runway overrun at Key West International Airport (EYW). The two pilots and three passengers were uninjured. According to the pilot, at touchdown the speed brakes extended but, when he went to apply wheel braking there was no braking. He then attempted to apply the emergency break but, there was still no breaking. Then the/ac overran the runway. 5 Un-injured Examination of the runway revealed that after the incident a/c had left 4,801 foot paved runway, it traveled into an engineered material arresting system (EMAS) which had been installed by the airport authority. The EMAS had decelerated the a/c to a stop in approx. 144 feet. *Preliminary NTSB Data
November 3, 2011 Fort Lauderdale, FL Cessna 172S N1855Y Around 1349 EDT, a/c was substantially damaged when it struck an idling Cessna 182T, while taxiing at Fort Lauderdale Executive Airport (FXE). No one on board was injured. According to FAA, the pilot started the engine, moved the airplane forward about five feet when testing the brakes, and then stopped. The Cessna 172 pilot was taxiing a tie-down spot and taxied in front on the Cessna 182. The outboard section of the left wing of the Cessna 172 impacted the propeller of the Cessna 182 causing substantial damage to the wing. 5 Un-injured *Preliminary NTSB Data
November 5, 2011 West Palm Beach, FL Robinson Helicopter R22 Beta N413RM Around 1620 EST, a/c was substantially damaged during an autorotation, following a loss of rotor RPM near West Palm Beach. The pilot and passenger incurred minor injuries. The pilot noticed a loss of airspeed after he turned it southbound. He then moved the cyclic forward in an attempt to maintain airspeed, however, the low rotor RPM horn sounded and accompanying cockpit indication illuminated. November 6, 2011 Lake Havasu City, AZ Cessna 182Q N4701N Around 1850 MDT, a/c collided w/hilly terrain near Lake Havasu City. The pilot and two passengers sustained fatal injuries. The a/c sustained substantial damage and was fragmented during the accident sequence. The a/c became the subject an alert notice (ALNOT) at 0930 on Nov. 9, after the pilot didn't report for work as expected. The Civil Air Patrol and Mohave County Sherriff's dept. subsequently initiated a search, and the a/c was located at 1600. 3 Fatal The flight departed Glendale Municipal Airport about 1800 w/ a planned destination of lake Havasu City Airport.
November 6, 2011 Yucca Valley, CA Beech A36 N64Z Around 0955 PST, a/c made a forced landing following a loss of engine power at Yucca Valley Airport. The pilot and the flight instructor were uninjured. The a/c sustained substantial damage to the right wing. The PUI reported that they were conducting a simulated loss of engine power ending w/ an airport landing. When he attempted to reapply power, there was no corresponding increase in engine power. 2 Uninjured The a/c was recovered for further examination. *Preliminary NTSB Data
November 10, 2011 Pukoo, HI Eurocopter EC 130 B4 N11QV Around 1214 HST, a/c collided w/ mountainous terrain near Pukoo. The pilot and four passengers were fatally injured. Witness reported that their attention was drawn to the helicopter when they heard some form of "woop wooping" sound. One witness observed the a/c descending from the ridgeline; he reported that he observed pieces falling from the descending a/c. 5 Fatal The flight originated from the Kahului Aiport about 1144. All witness reported rain showers in the area during the time frame of the accident. Several witnesses reported that the accident occurred between rain "squalls" and one reported that it occurred during a heavy rain "squall." *Preliminary NTSB Data
November 11, 2011 Mexico City, Mexico Eurocopter AS332L XC-UHM A/c crashed into a rural area south of Mexico City enroute from Camp Marte, Mexico to Cuernavac, Mexico. The eight passengers and crew were fatally injured. 8 Fatal The investigation is being conducted by the Mexican Direccion General de Aeronautica Civil (DGAC).
November 14, 2011 Cayman Brac, Cayman Islands Cessna 210 XB-LLD Around 0525 UCT, a/c was substantially damaged when it struck wires and tress under unknown circumstances at Cayman Brac, Cyaman Islands. The pilot and passenger were fatally injured. 2 Fatal *Factual NTSB Data
