A/c substantially damaged when struck trees and house near Jackson/Hawkins Field (HKS). Attention drawn to a/c by its noise, which witness described as repeated "clup" sounds. One fatality, one serious Witness saw a/c descend rapidly towards him from N, strike a tree, and enter "into a spin." It then struck a second tree and "fell into the house," disappearing from view.
Sept 12, 2009 Lake Elsinore, CA SCHWEIZER SGS 1-34 N7656 A/c stalled on takeoff and impacted ground in nose down attitude at Skylark Field Airport (CA89). Tow pilot said prior to liftoff and traveling about 65 mph had to manipulate a/c due to crosswind or glider moving. One fatality After takeoff felt "slack in tow line," and tried to slow down via backpressure on flight controls when felt "surge" forward. Pilot looked back and saw glider in a sharp turn. Pilot noted that altitude was between 225 to 250 feet at time. Witnesses said glider released from tow plane, and then veered sharply to right before entered into nose dive prior to impacting ground.

A/c operated by Geliks Airline crashed 6.2 miles from Perm Airport. Two fatalities Pilot and passenger killed and a/c sustained substantial damage.
Sept 15, 2009 Castro Verde, Portugal PIPER PA34 OO-TML A/c registered to Ben-Air Flight Academy destroyed upon impact with terrain during night navigation training flight. Three fatalities A/c had disappeared from radar and no communication contact established. Flight instructor and two students fatally injured. Wreckage spread over large area.
Sept 16, 2009 Redlands, CA QUAD CITY CHALLENGER II N732LS A/c impacted terrain in citrus grove. Witness observed high wing, 'ultralight' airplane at elevation of 300-500 feet agl at a position that corresponded to left base of runway 26. Two fatalities Observed a/c descend at 45-degree angle behind trees and out of his field of vision.
Sept 19, 2009 Reno, NV CASSUTT III M C-FDXO A/c made forced landing at Reno Stead Airport. Local air race flight. Visual meteorological conditions prevailed, and no flight plan had been filed. Pilot's crew chief said during race two days prior to accident, oil line separated, and engine overheated. Maintenance crew could hear synthetic oil boiling, and observed globs of oil being ejected from engine breather tube. It took engine about 6 hours to cool so they could work on it. On serious Drained remaining oil and checked oil screen, which contained no debris. Borescoped cylinders and noted no damage. The crankshaft rotated freely. During accident race, pilot reported that engine was overheating, but turning. Elected to land on runway 14. As on base leg to runway, engine stopped rotating, and a/c began sinking rapidly. It landed hard about 50 yards from approach end of the runway.
Sept 20, 2009 Forest Grove, OR ROBINSON HELICOPTER R22 BETA N956SH A/c collided with terrain near Forest Grove, Oregon. Operator reported that PUI was preparing for CFI check ride. Two airmen going to practice autorotations with power recoveries. Two fatalities Witness observed several uneventful autorotations, and then stopped watching. Another witness observed a/c descend rapidly until collided with ground.
Sept 20, 2009 Valley Center, CA ROBINSON HELICOPTER COMPANY R44 II N4174P A/c collided with terrain during a forced landing. As approached landing zone at altitude of between 50-75 feet agl, pilot heard popping sound followed by "silence." Two serious Nose of a/c dropped, and pilot assumed engine had failed. Lowered collective and performed autorotation. During landing sequence a/c sustained substantial damage to fuselage and tailboom became separated midspan.
Sept 22, 2009 Columbia, MO PIPER PA-12 N3234M A/c sustained substantial damage when impacted terrain about five miles E-NE of Columbia Regional Airport (COU). One fatality Personal flight had departed Montgomery-Wehrman Airport (4MO), Montgomery, Missouri and was en route to Wichita, Kansas.
Sept 22, 2009 Blackpool, UK SCHWEIZER 269C G-LINX A/c registered to Heli-Lynx was destroyed when impacted terrain. Two fatalities Visual meteorological conditions prevailed at time of accident. Pilot and passenger fatally injured. Accident flight originated from Blackpool.
Sept 24, 2009 Tucson, AZ EUROCOPTER AS 350 B3 N103LN Air Methods chopper experienced loss of tail rotor authority and landed hard on hospital landing pad, St Mary's Helistop. Pilot said during final phase of landing a/c was 5 feet over landing pad at St Mary's Hospital when started to yaw left in uncommanded fashion. No injuries Pilot countered yaw with right antitorque pedal, which had no effect. He then rapidly lowered collective, landed hard, and a/c came to rest facing opposite direction.
Sept 24, 2009 Nampa, ID Frazier Tangent N72TZ A/c collided with terrain during high speed taxi tests at Nampa Municipal Airport. One fatality Witnesses said pilot performing high speed taxi tests and routinely raised nose of a/c up during test runs. During test run that precipitated the accident, a/c became airborne and rose to 75 feet, started right-hand turn, stalled, and impacted terrain in flat attitude. Audible engine sounds could be heard from a/c throughout event.

One fatality, one serious The accident occurred about 30 kilometres (20 miles) east of the coastal city of Perpignan during a test flight in which the two aircraft were taking part.
Sept. 27, 2009 Incheon, South Korea unavail A light plane has crashed at the site of an international fair packed with thousands of visitors in South Korea, killing one of its two pilots and injuring eight people on the ground, The plane hit an empty bus parked at the site in Incheon, just west of Seoul, One fatality, nine injuries About 20,000 people were at the fair site at the time of the crash. He said the plane was flying as part of a separate city festival.
