July 11, 2009 Masizo del Roraima, Venezuela BEECH A36 N354RA The wreckage was located on the "Ilu - Tepuy" mountain, within the sector of Masizo del Roraima, Bolivar, Venezuela. Pilot had filed a flight plan from Maurice Bishop International Airport, Grenada to the Boa Vista International Airport, Brazil. Last radio contact was with Venezuelan Control Center, in which reported position as 63 nautical miles N of Santa Elina de Uairen, Venezuela, July 11, at 1813. Two fatalities Authorities from both Brazil and Venezuela involved in search and rescue operation once a/c reported overdue. A/c impacted side of mountain at estimated 8,200 feet AGL. Pilot and passenger were killed, and a/c was destroyed.
July 15, 2009 Charlotte, NC BOEING 737-3B7 N506AU US Airways tech performing walk around check post Flight 1783. Technician noticed hydraulic fluid leaking through pinhole in Left Main Landing Gear Outer Cylinder. A/c was removed from service for further investigation and repair. No injuries Discrepant landing gear was subsequently replaced, sent to AAR Landing Gear Services for initial assessment, and later sent to Boeing for more detailed analysis.
July 20, 2009 Tajima, Japan ROBINSON R44 JA32CT Robinson R44, Japanese registration, collided with mountainous terrain nine miles SE of Tajima Airport. Two fatalities A/c sustained substantial damage due to impact forces. Pilot and one passenger died at scene.
July 24, 2009 Melbourne, Australia BOEING B37-700 VH-VBA Virgin Blue Boeing 737-700 suffered failure of nose landing gear axle. During taxi for takeoff, right nosewheel separated from nose landing gear leg. A/c able to taxi back to gate. Initial examination of landing gear indicates region of fatigue cracking in lower section of landing gear axle, which subsequently failed.
August 04, 2009 Jacksonville, FL EUROCOPTER AS 350B3 N186AE A/c substantially damaged while maneuvering at Herlong Airport (HEG), Chopper owned by US Department of Homeland Security and operated by Customs and Border Protection (CBP) as Public Use flight. CBP representative said pilots practicing maneuvers with hydraulic system turned off, to simulate hydraulic system failure. One minor Pilot conducting practice approach to runway 25, with transition to hover, when a/c contacted ground, spun, and rolled over. A/c came to rest on S side of runway. Fuselage, tail rotor and main rotor drive systems substantially damaged.
August 05, 2009 Mont Laurier, Canada ENSTROM F28 C-GVQQ A/c destroyed when impacted wires and terrain near Mont-Laurier Airport (CSD4), Two fatalities Visual meteorological conditions prevailed at time of accident. Pilot and passenger fatally injured. Flight originated from CSD4.
August 05, 2009 Napa, CA CESSNA 182S 23750 A/c impacted hill shortly after departing from Napa County Airport. Accident site in Napa Valley hills about 3.25 nm SE of departure end of runway 18R. Main wreckage came to rest at edge of vineyard, with inboard section of left wing and tail section near remnants of burned fuselage. One fatality Fragments of a/c's wheel pants were farthest debris found from main wreckage; located downslope 250 feet on median magnetic bearing of about 345 degrees. All control surfaces accounted for at accident site.
August 07, 2009 Adrian, OR NAVION A N4898K A/c destroyed during in-flight breakup near Adrian. Pilot, who had recently purchased a/c. Wreckage debris scattered over 1,000-foot area of hilly terrain. Three fatalities At far S premise of debris field were large sections of right wing, with far N area containing engine and nose landing wheel.
August 08, 2009 Keymar, MD Kociemba Robert H T-Bird II N90458 A/c substantially damaged during forced landing near Keymar, MD, shortly after takeoff. Witnesses reported to FAA inspector that observed flight departing and then engine "stopped." An on-scene examination of a/c by FAA revealed Two fatalities that wooden propeller had little damage and was devoid of chordwise scratches to indicate any rotation prior to accident. Aileron and elevator control cables exhibited corrosion. Fuel found throughout fuel system, however contaminants were also present.
August 08, 2009 Hodgdon, ME Tidd Wesley Mini-MaxN569WT A/c substantially damaged during forced landing. Witnesses reported hearing and visually acquiring a/c. One witness reported to Maine State Police that engine began to sputter and then stopped. One serious Witness continued to watch a/c as began approach into field located behind residence. Saw a/c land in field and nose over, coming to rest on top of vertical stabilizer as well as canopy.
August 12, 2009 Eden Prairie, MN BEECH E18S N3038C A/c collided with terrain following loss of control while circling Flying Cloud Airport (FCM). Witnesses saw a/c depart FCM and climb to altitude of 500 feet agl or less. A/c made left hand turns until parallel to N side of runway 28R in direction of takeoff. Two fatalities Witnesses said a/c low and slow from time it took off. Described a/c as yawing and banking to left and right in nose high attitude throughout flight. Left wing dropped and nose descended just prior to a/c impacting terrain.
August 15, 2009 Caddo Mills, TX CESSNA 182 N2364G Rotating propeller struck and injured deplaning passenger at Caddo Mills Municipal Airport (7F3), Flight conducting parachute jump operations. One serious A/c stationary on taxiway with engine operating. Pilot speaking with person who had come to assist passenger. Passenger exited right door of a/c and walked forward toward waiting vehicle when struck by propeller.
August 16, 2009 Tea, SD FROM HERE TO THERE PDQ RV-8N98AW A/c collided with terrain while performing aerobatic maneuvers. Witness reported seeing a/c perform and recover from hammerhead-type maneuver. Two fatalities Second maneuver performed from which a/c did not recover. Witness said engine sputtered and power sounded like went to idle as a/c spun toward terrain.

Iranian AF helicopter Crashed near Tehran. Crash was latest added to poor air safety record of Iran, which has seen series of aviation disasters over past month.Came one day after small training aircraft crashed and burst into flames near Tehran, killing two people on board. Three fatalities On Aug. 10, three people killed and three others injured in Bell 205 police helicopter crash in southern Iran.
Aug. 16, 2009 Zhukovsky airfield, east of Moscow Su-27 fighters Two Russian AF a/c rehearsing aerobatic maneuvers collided near Moscow, killing one stunt pilot and sending one fighter crashing into nearby vacation homes. One fatality, two serious Su-27 fighters were part of the elite Russian Knights flying group preparing to perform at the MAKS-2009 air show. Three pilots involved ejected.

UK Military Chinook helicopter Crew of British military helicopter in Afghanistan escaped without injury but their aircraft was destroyed after a possible Taliban attack. No injuries The Chinook helicopter made an emergency landing after an "engine fire" in the violence-torn southern Helmand province.

Cessna 206 Three people emerged terrified but happy to be alive after their plane made an emergency landing on the busy Kampala-Masaka highway. Plane, with two Portuguese passengers, traveling from Kisoro Gorilla Mountain Park to airbase in Kajjansi, near Kampala. No injuries A/c belonging to Kampala Aero Club Flight Training Centre, made emergency landing in middle of oad at Kyabaddaza, about 40km W of Kampala. A/c had run out of fuel, according to Civil Aviation Authority. Emergency landing was result of "excessive fuel venting from a fuel cap exacerbated by unreasonably bad weather in the area."

U.S. Army UH-60 Black Hawk from Fort Campbell, KY on training exercise when it crashed 400 feet from summit of 14,421-foot Mount Massive. four fatalities Cause of crash wasn't immediately released. Flight recorder was recovered. Chopper assigned to 160th Special Operations Aviation Regiment (Airborne) 160th known as "night stalkers" because they specialize in nighttime operations.
