July 08, 2009 Gulf of Mexico CESSNA 421C N4467D A/c destroyed after impacted waters of Gulf of Mexico, 25 nautical miles NW of Port Richey, FL. Five fatalities Pilot was in communication with Jacksonville ARTCC just prior to accident. Pilot said a/c encountered turbulence, and the final transmission indicate that a/c was "upside down." Moments later, radio and radar contact was lost as a/c descended through 6,000 feet.
July 16, 2009 Morristown, TN CESSNA 120 N7227E A/c nosed over during forced landing on golf course near Moore-Murrell Airport (MOR). One minor Preliminary examination of a/c by FAA inspector revealed evidence of fuel leakage from right wing and left wing.
July 17, 2009 Fox Lake, IL CESSNA C-182 N7482Q Pilot moving float equipped a/c from pier to other side of lake. He decided to fly to other side instead of performing water taxi. No injuries During climb out, encountered heavy rain showers, which obscured forward visibility out windshield. Executed downwind landing to lake by looking outside window. During landing, could not slow a/c down enough to avoid hitting pier.
July 17, 2009 Hayesville, NC CESSNA R182 N778RK A/c destroyed when impacted trees and terrain. Review of prelim radar data revealed that visual flight rules (transponder code 1200) target was acquired just N of 0GE5. Target tracked generally northbound, then turned about 30 degrees left, directly toward RHP. One fatality Target gradually descended from 5,600 feet to 4,700 feet, where last radar target was observed about 27 nm SE of RHP.
July 18, 2009 La Porte, TX FAULK MARVIN Faulk N62431 A/c crashed during takeoff on N edge of the La Porte Airport (T41), A/c impacted tree and crashed in back yard of residence. One serious Witness saw a/c during takeoff from runway 31 and said engine was running. At 200 feet AGL they observed a/c descending while in "a high angle of attack" and a/c continued in that attitude all the way to impact.
July 18, 2009 Perrigny, France CIRRUS SR22 N752CD A/c owned by AeroColor Inc., Las Vegas, NV destroyed when impacted terrain under unknown circumstances near Perrigny. One fatality Pilot was fatally injured. The flight originated from Luxembourg (LUX), Luxembourg.
July 18, 2009 Ponce, PR CESSNA 152 N5518P A/c lost at sea in vicinity of Ponce, Puerto Rico. Pilot fatally injured. One fatality No low-level radar coverage of area. FAA issued an Alert Notice (ALNOT), and on July 20, pilot's body found near mouth of Rio Inabon, near Ponce. Several days later, a wheel from a/c was found.
July 19, 2009 Columbus, GA James O'Bert Vari-Eze N39DX A/c substantially damaged when collided with buildings and tractor after takeoff at Columbus Regional Airport (CSG). One fatality Pilot practicing takeoffs and landings. According to prelim info provided by FAA, pilot of accident a/c radioed he "had a problem." A/c "veered to the right," "clipped a hangar," "skipped" off top of a shed, and then impacted an unoccupied tractor and broke apart.
July 22, 2009 Franklin, PA HUGHES 369 N61735 A/c substantially damaged during forced landing in open field in Forbes State Forest, Flight flew to pre-determined locations to retrieve 4 bags carry seismic monitoring equipment. One fatality During flight's return to landing zone, personnel heard pilot talking over their communication frequency, however most of communication was inaudible but some heard pilot saying that he was "going into the tree."
July 24, 2009 Dunedin, FL ROBINSON R44 II N645JC A/c sustained main rotor blade damage during autorotative landing into Gulf of Mexico. Pilot applied forward cyclic control to increase indicated airspeed and banked to left. Saw low main rotor warning light, and banked to right flying towards shore. No injuries Heard main rotor low warning horn. Intended on descending to get into ground effect. When a/c between 30-40 feet, flared (applied aft cyclic control), and while descending pulled collective control, which was full up at or just before water contact. Applied left cyclic control to stop main rotor blades. Occupants exited a/c.
July 30, 2009 Ravenswood, WV CIRRUS SR-22 N581DS Preliminary info from FAA & data downloaded from Recoverable Data Module (RDM), flight was cleared by ATC to climb to 25,000 feet. Flight was cleared to climb from 22,000 feet and to maintain 23,000 feet. Pilot acknowledged instruction, but controller noted that his voice had changed, and had taken on a "helium/Mickey Mouse" quality. ATC attempted to contact pilot for approximately 6 minutes before pilot responded with, "Go ahead." A/c issued clearance down to 12,000 feet and instructed to "begin your descent." One fatality Airline pilot on frequency commented that pilot sounded "incoherent." Last radio transmission received was "pilot's labored breathing." Ohio ANG launched F-16s. Pilots unable to gain pilot's attention visually or by radio. They noted that pilot appeared "unconscious" at flight controls. Air Force jets remained with a/c until departed controlled flight, and descended into terrain.
July 30, 2009 Norridgewock, ME Christopher John P CHR-1 N179TC Experimental amateur built gyrocopter was substantially damaged following forced landing at Central Maine Airport (OWK). No injuries Pilot said after takeoff climbed to pattern altitude and while on downwind and abeam the runway numbers heard a "pop and then experienced a terrible, violent vibration. Over landfill, which was closest site for emergency landing.
August 01, 2009 Nairobi, Kenya CESSNA U206 N5Y-LMB A/c owned and operated by African Inland Mission collided with ground at Nairobi. A/c conducting low flight while passengers filming underlying area. One fatal, one serious During accident sequence, a/c struck power line and building.
August 03, 2009 Torrance, Canada Cessna TU-206 C-GGMG Impacted trees during takeoff from Lake Muskoka. Two fatalities A/c destroyed. Pilot and passenger sustained fatal injuries. Flight originating at time of accident.
August 03, 2009 Bavaro, Dominican Republic BOEING 767-224 N76156 Continental Airlines Flight 128 enroute from Rio de Janeiro, Brazil (GIG) to Houston Intercontinental Airport (IAH), Houston, TX, encountered turbulence at an altitude of 36,000 feet. 33 minor Flight crew declared emergency and diverted to Miami International Airport (MIA), Preliminary examination of satellite weather imagery around time of event indicates presence of isolated, rapidly developing cumulus congestus to cumulonimbus clouds under flight path.
August 04, 2009 Louisville, KY BOMBARDIER CL-600-2B19 N8543F Pinnacle Airlines CRJ-200 operating as Flight 2871 enroute from Knoxville, TN to Detroit, MI when flight encountered turbulence. Flight had been operating at FL300 and had just initiated a climb over cumulonimbus clouds when turbulence was encountered. One serious Flight attendant was transported to local hospital and passenger was treated at airport and released.
August 04, 2009 Happy Jack, AZ BOEING E75 N5195N A/c substantially damaged when impacted terrain while maneuvering. One fatality, one serious Passenger said after departure, a/c had difficulty gaining altitude during cruise flight. Pilot decided to turn back in attempt to gain altitude. Passenger was looking at sectional map attempting to verify terrain elevations in area of intended flight path when blacked out.
August 07, 2009 Gladwin, MI HOOPER JAMES A BUSHBY MUS N37JD A/c destroyed when impacted trees and terrain while maneuvering. One fatality According to prelim radar data, a/c departed 3BS from runway 06 and made turn to N. A/c continued N and made a series of 3 turns between 2,500 and 2,700 feet msl. A/c Made turn back toward 3BS and shortly thereafter radar contact lost.
August 07, 2009 Saint Charles, ID PIPER PA-22-150 N6905B A/c destroyed when impacted terrain, Two fatalities A/c impacted open field within heavily wooded mountainous area. A/c came to rest in upright position and was consumed by post-crash fire. All major structural components located at accident site.
August 08, 2009 Calhoun, TN Stump Bradley Kolb N2456 A/c substantially damaged following loss of control inflight. Two fatalities Witness observed a/c climb straight up and then do "cartwheel" and come straight down then level out. A/c did this maneuver three times; however, on third time a/c went straight up and "flipped over backwards, started spinning, and then collided with ground".
August 09, 2009 Bumpass, VA MAXAIR Drifter N160HA A/c substantially damaged following loss of control after takeoff from surface of Lake Anna. Witnesses described application of full power and described steep left bank almost immediately after takeoff before a/c descended nose down into water, and submerged. Witness "alarmed" by how close to shore a/c was before it lifted off water. One fatality He said, "as the aircraft banked left along the shoreline to head out into open water, I saw the aircraft attempt to make what looked like a very sharp turn to avoid the approaching tree line."
August 10, 2009 Tokyo, Japan BOEING 737 JA56AN A/c operated by Air Nippon Airways as Flight ANA298 took off from Tottori Airport and landed at Runway B of Tokyo International Airport. No injuries Post flight check showed that lower tailpiece of fuselage damaged about five meters long and one meter wide. Two scratch marks (about 25 meters long and 7.5 meters long) made on Runway B of Tokyo International.
Aug. 11, 2009 Papua, New Guinea unavailable Airlines PNG chartered plane crashed en route to tourist region of Papua New Guinea. 13 fatalities Plane carrying 11 passengers and two crew, vanished in bad weather on approach to airport in Kokoda region.
