| March 02, 2009 Galliano, LA | BELL 206 N401RL | A/c damaged when tail rotor drive shaft and coupling severed while a/c was sitting on ramp with engine running. | No injuries | Pilot heard loud bang and vibration shortly after starting engine. Shut down engine and exited a/c. Examination of a/c revealed that tail rotor drive shaft and coupling had severed just forward of gearbox, which resulted in substantial damage to tail boom. |

Able to slow a/c and exit runway onto taxiway. Upon opening cabin door, observed fire near left main landing gear and evacuated. |

The pilot applied spin recovery controls and the airplane continued to spin without apparent response, so the pilot deployed the airplane's Ballistic Recovery System (BRS) parachute. |
