Witness heard engine of accident a/c "revving" and then observed a/c "going in a circle." After a/c went behind row of trees witness heard engine surge followed by rising smoke from area. One eyewitness heard owner inform pilot "not to go up because of the wind." |

Purpose of flight was to fly to Humboldt for transfer of patient from Humboldt General Hospital, Winnemucca, NV to Renown Regional Medical Center. | No injuries | Pilot took off from Renown helicopter pad and positioned a/c in to a high hover, 25 feet, per the normal operating procedures. Just as beginning transition to forward flight heard loud bang, and a/c experienced partial power loss. |

Witness reported that heard radio transmission on Unicorn frequency and was looking to the W to observe a/c as it landed. Saw a/c as it descended out of clouds. Was in nose low attitude, rolling to L into steep left turn with wings at almost a 90 degree position relative to ground. | Four fatalities | A "huge cloud of snow" was created by subsequent impact and when it cleared, observed wreckage at W end of airport property. Happened very fast - "in the blink of an eye. " Said landing light not turned on. |

A/c departed runway 09 at ARR. A/c issued left turn to heading of 270 and instructed to contact departure control. | Two fatalities | Pilot checked in with departure control reporting that leaving 1,300 feet climbing to 3,000 feet. Controller then cleared pilot to climb to 4,000 feet. Pilot acknowledged clearance. There was no further contact between air traffic control and pilot. |
