Dec 03, 2008 Rio Grande, PR ROCKWELL INTL 690B N318WA A/c destroyed when impacted terrain while maneuvering about four nm SE of Rio Grande, PR. Witnesses reported that in area surrounding accident site fog and rain were present and mountains were obscured from view. Three fatalities A/c observed by approach controller to deviate to W-SW from assigned heading towards mountainous terrain.
Dec 11, 2008 Sabine Pass, TX BELL 206 N180AL A/c owned and operated by Rotorcraft Leasing Company destroyed after impacting water eight miles S of Sabine Pass, TX in the Gulf of Mexico. Five fatalities Flight following services and company policy requires 15 minute position report for every flight. Pilot did not make required 15-minute position report and search and rescue efforts initiated.
Dec 11, 2008 Greer, SC MD-88 N924DL A/c operated by Delta Air Lines as Flight 1102, diverted to Greenville Spartanburg International Airport (GSP) after experiencing smoke in cockpit while on flight from Washington Dulles International Airport (IAD) to Atlanta Hartsfield International Airport (ATL). No injuries Smoke appeared to be emanating from area of co-pilot's glare shield and had electrical odor. Flight crew donned oxygen masks and goggles, accomplished "Smoke/Fumes Checklist", declared emergency, and diverted to GSP where a landing and an emergency evacuation was performed.
Dec 13, 2008 Sebring, FL ERCOUPE 415-D N99154 A/c destroyed during in-flight breakup. Two fatalities Witness observed a/c flying overhead, initially flying SW, and made a "very slight dip and turned to the right. A/c pulled up severely and started turning to the left." As a/c continued upward and banked to left, something "shiny" exited from tail area.
Dec 14, 2008 Philadelphia, PA BOMBARDIER CL-600-2B19 N407AW Air Wisconsin Airlines Flight 3919 landed at Philadelphia International Airport (PHL) with left main landing gear in retracted position. No injuries Maintenance had been performed on both the left and right main landing gear systems prior to incident flight.
Dec 18, 2008 Louisville, KY BEECH 36 N7472N A/c crashed in open field within golfing community during instrument flight rules (IFR) flight from Chicago Midway International (MDW), Chicago, Illinois, to Bowman Field Airport (LOU), One fatality Controller from Louisville International Airport-Standiford Field (SDR) cleared pilot when a/c was about 9 miles out, for the Area Navigation (RNAV) Global Positioning System (GPS) approach for Runway 24 at LOU. Pilot communicated an emergency advising engine problem.
Dec 18, 2008 Buenos Aires, Argentina PIPER PA-31T LV-MYX A/c operated by Aviacenter impacted ground few minutes after takeoff from San Fernando Airport (SADF). Two fatalities Visual flight rules prevailed. A/c was substantially damaged. Two pilots were killed.
Dec 20, 2008 Aguanga, CA BEECH V35 N1587L A/c substantially damaged following impact with terrain while maneuvering near Aguanga. Two fatalites Examination of wreckage revealed that a/c had impacted open field in nose low, wings level attitude.
Dec 26, 2008 Kansas City, MO EMBRAER ERJ 170-10 N871RW A/c encountered severe turbulence while en route to Kansas City International Airport (MCI). One serious One flight attendant sustained serious injury. Rest uninjured. A/c owned and operated by Republic Airlines.
Dec 30, 2008 Pittstown, NJ PIPER PA-28-140 N95156 A/c damaged when impacted trees and terrain while on final approach to Alexandria Airport (N85). One fatal, one serious While pilot turning a/c from downwind leg to base leg, a/c began striking trees.
Dec 31, 2008 Panama City, FL SCHWEIZER 269C N36254 Pilot conducting sightseeing flight, looking for alligators, and he entered an out-of-ground-effect hover at 50-70 feet agl. No injuries Hovered the helicopter into the wind, which he based on the water conditions. After hovering for about 5 seconds, the pilot pitched forward on the cyclic and raised collective. A/c entered uncommanded right yaw.
Jan 02, 2009 Molesmes, France Eurocopter EC 135 T2 F-HBM A/c owned by Regourd Aviation destroyed when impacted terrain under unknown circumstances. One fatality Pilot fatally injured. Flight originated from Issy-Les-Moulineaux (LFPI), Paris and was en route to Lyon Bron, France.
Jan 04, 2009 Morgan City, LA SIKORSKY S-76C++ N748P A A/c operated by Petroleum Helicopters (PHI), crashed into marshy terrain approximately 7 minutes after takeoff and 12 miles SE of departure heliport. 8 fatalities, one serious Flight was being tracked by satellite based system. Departure from 7LS3 was reported at 1402. The track suddenly ended about 7 minutes after departure at 1409. No reports of any problems from the flight crew on PHI radio frequencies or emergency transmissions on any monitored air traffic control frequencies.
Jan 05, 2009 Milford, IN GRUMMAN AA-5B N28862 Grumman Tiger substantially damaged following in-flight separation of propeller blade and forced landing. No injuries Pilot said a/c was in cruise flight at 3,500 feet msl when heard a "loud bang followed by a violent vibration."
Jan 07, 2009 Iola, KS LAVIGNA DANIEL M JR LANCAIR IV P, N131DL Experimental a/c destroyed following loss of engine power and subsequent hard landing at Allen County Airport (K88). One serious While in landing flare, pilot observed a/c on short final landing in opposite direction. Pilot elected to abort landing and applied full engine power. As a/c began to climb, engine stopped producing power.
Jan 11, 2009 Hayden, CO PILATUS PC-12/45 N604 A/c destroyed on impact with terrain following loss of control while on initial takeoff climb. Two fatalities Line crew reported "heavy" snowfall from time a/c was pulled from hangar until it departed. Crew members reported seeing accumulation of "wet snow" on a/c's wings.
Jan 12, 2009 Kilgore, TX Probst Baby Ace D N610DD WP A/c substantially damaged when impacted terrain five miles N-NE of Kilgore. One fatality No witnesses to impact. Wreckage discovered by local resident.
Jan 16, 2009 Mauna Loa, HI GLASER-DIRKS DG-400 N777WN Motor glider collided with mountainous terrain on SW slope of Mauna Loa Mountain, Hawaii. One fatality Pilot attempting to set record. Pilot's glider towed to 12,300 msl and released. Pilot radioed that he was in the primary wave, climbing at 1,000 feet per minute, and was at 28,000 feet (msl). Then stated was going to crossover to Mauno Loa.
Jan 19, 2009 Konigstein, Germany PIPER PA-42-720 DIDIA A/c owned by HELI Flight Flugschule Flugbetrie destroyed when impacted terrain under unknown circumstances near Konigstein, Hessen, Germany. One fatality Flight originated from Frankfurt/Main, Germany, a nd en route to Bad Nauheim, Germany.
Jan 22, 2009 Naples, FL Cessna 402C Flight 9399 en route from Key West to Southwest Florida International Airport in Fort Myers when reported loss of power in its engines and diverted to Naples Municipal Airport. No injuries A/c glided to safe landing. Six people and pilot on board. Nobody injured and no other damage to a/c.
Jan 24, 2009 Oceanside, CA CESSNA 172S N562AD A/c collided with ground following loss of engine power during go-around at Oceanside Municipal Airport. Two serious Passenger reported that while they were in cruise flight over Palomar, CA, at 6,000 msl, pilot noticed that fuel gages were going down fast. Pilot thought that a/c leaking fuel. Diverted to Oceanside and attempted to land on Runway 24.
Jan. 27, 2008 Lubbock, TX ATR-42 N902FX Empire Airlines Flight 8284, operating FedEx-owned ATR-42 crashed 300 feet short of threshold while on ILS approach to Runway 17 at Lubbock International Airport. No injuries A/c destroyed by impact and post-impact
