Sept 01, 2008 Bukavu, Congo Beech 1900C ZS-OLD A/c operated by Air Serv International collided with high terrain during approach to Bukavu, Democratic Republic of Congo (DRC). 17 fatalities Pilot, copilot, and 15 passengers fatally injured. The airplane was destroyed. The last departure point was Kisangani, DRC.
Sept 07, 2008 Baxter Pass, CO Gulfstream American AA-5A Cheetah N26930 A/c impacted terrain while conducting a pipeline patrol near Baxter Pass, CO. Two fataliies A/c was last seen flying north at low altitude. Witnesses stated the weather was clear, temperature between 70-80 degrees, and winds were calm.
Sept 14, 2008 Wichita Falls, TX American Champion (ACAC) 7GCBC N571JH A/c substantially damaged upon impact with terrain near Kickapoo Municipal Airport (CWC). Witness heard engine surge loudly and saw a/c" struggling to find lift." Another person heard engine "popping," go silent, and starting producing a "popping" noise again. One fatality Witnesses observed a/c's base to final leg and heard no engine noise. A/c was observed at approximately 50 feet AGL and slowly descending. As the a/c approached open field near church, witnesses reported that a/c pitched nose up before sharply rolling right and impacting ground.
Sept 19, 2008 York, NE CESSNA 172 N4380Q A/c registered to/operated by York Aviation substantially damaged when the struck runway during uncontrolled descent shortly after taking off from York Municipal Airport (JYR). According to airport manager, flight instructor and student pilot were planning to do touch-and-go landings. He came out of hangar and noticed wreckage in distance. Drove to accident site and called 911. Two fatalities On-scene examination revealed a/c struck runway to left of centerline at 3,000-foot mark. Measurements indicate impact occurred in left wing low, nose low attitude. A/c then skidded 182 feet off the left side and into the grass.
Sept 24, 2008 Hoxton Park, Australia Liberty Aerospace Inc. XL-2 VH-CZX A/c collided with the terrain near Hoxton Park, Australia. One fatality The pilot, the sole occupant, was killed. It is reported that the airplane collided in an open level field.
Oct 01, 2008 Vero Beach, FL Bezinque Alisport-Silent N373DB Experimental amateur-built glider, was substantially damaged during forced landing after takeoff from Indian River Aerodrome (FL74) One serious Pilot turned glider to return to airport. When he leveled the wings, the engine stopped producing power. Lacking the altitude to return to airport, pilot chose a road for forced landing. The landing area was blocked with cars and power poles, so he landed the glider "hard" in the grass adjacent to the roadway to avoid the obstacles.
Oct 01, 2008 Blaine, WA EUROCOPTER AS350 N172AE A/c substantially damaged during hard landing at Blaine Municipal Airport. The a/c registered to US Department of Homeland Security and operated by US Customs and Border Protection. Pilot and flight instructor were conducting series of practice emergency procedures during annual proficiency evaluation flight. After completion of two landings, instructor said going to conduct manual governor operations and positioned the governor switch from "auto" to "man" while on the downwind leg to Runway 32. No injuries After performing left 360-degree turn, pilot initiated normal approach to runway with intention of performing run-on landing. As a/c descended through 20 feet agl, attempted to increase throttle and collective to arrest rate of descent when it "felt like the bottom fell out" and a/c landed hard on asphalt runway surface in slight nose high attitude.
Oct 02, 2008 Bangkok, Thailand BOEING 747 B-18202 China Airlines Boeing 747-400 registered in Taiwan experienced turbulence while enroute to Bangkok from Hong Kong International Airport. One serious The flight was at flight level 400 and 128 nm NE of Bangkok at time of event. Ten passengers and four cabin crewmembers sustained injuries. One passenger suffered spinal cord injury.
Oct 03, 2008 Chugiak, AK Antonovich Challenger II N7511J A/c damaged when collided with tree-covered terrain following loss of control while maneuvering about five miles NE of Chugiak, Alaska. Witnesses said that just before accident, they saw a/c flying northbound, about 100 feet above trees. One fatality Witness said that as a/c continued northbound, engine noise increased, and nose pitched down. Witness said a/c's nose continued to pitch down until a/c became inverted, and descended nose first into dense woods.
Oct 05, 2008 Lakeland, FL CIRRUS SR22 N101CD A/c experienced asymmetrical flap extension while approaching Lakeland Linder Regional Airport (LAL). After entering traffic pattern, pilot slowed a/c to flap extension speed, and set the flaps to 50-percent position. Upon moving flap selector to mid-position, pilot heard a "bang" and a/c rolled to right. Pilot then responded with "full roll input," and contacted air traffic control to advise them of problem. No injuries Pilot looked outside and noticed that left flap fully extended, but could not see the right flap's position. Pilot subsequently landed a/c uneventfully using "full roll input." After exiting a/c, pilot observed left flap in fully extended position and right flap in fully retracted position.
Oct 05, 2008 Sandy Valley, NV Bonde TF-51D N267B A/c substantially damaged when impacted terrain while maneuvering near the Sky Ranch Airport (3L2). According to witnesses, pilot unable to extend left main landing gear after departure and conducted series of low approaches over runway while talking via radio to people located on ground. After conducting four passes over runway, pilot was able to extend left main landing gear about two-thirds the way down. One fatality Pilot continued to circle around the airport within the traffic pattern. Witnesses reported that while the airplane was on downwind for runway 21 at an altitude of about 400-500 feet above ground level, the airplane "appeared to enter a slip" followed by an immediate roll to the right into a nose low attitude and descended into terrain.
Oct 07 2008 Malibu, CA INIZIATIVE INDUSTRIALI ITALIAN SKY ARROW 600 N454SA A/c impacted ocean near Malibu, CA. Northfield Aviation LLC was operating the airplane. Two serious Witnesses observed a/c flying low over water. While making steep left turn, saw it nose over and impact ocean.
Oct 08, 2008 Weatherford, TX MD Helicopter MD 900 N902RN A/c experienced violent yaw to left while in cruise flight. No injuries Pilot regained directional control and continued on to KGPM and landed without further event or incident. Examination of a/c revealed that vertical stabilizer control rod had separated from actuator.
Oct 11, 2008 Santa Teresa, NM MAULE MXT-7-180A N4257C A/c impacted terrain shortly after takeoff from Dona Ana County Airport (5T6). The two pilots departed Glendale, Arizona (GEU) and arrived at 5T6 the day of accident. Witnesses saw a/c taxi and perform intersection takeoff on runway 28 directly towards isolated thunderstorm. Two fatalities The a/c appeared to climb to 150 to 200 feet above the ground before disappearing from view in the storm. Witness said a/c wings rocking back and forth and pilot appeared to be having difficulty controlling a/c just before it went out of view. Five to ten minutes later smoke was seen rising one mile from airport after storm moved past.
Oct 12, 2008 Mohawk, TN Sabre Tukan N75604 A/c substantially damaged following uncontrolled descent after takeoff from Cooper Field. Witnesses said strong, gusting wind from the east, but pilot elected to takeoff to the west. During takeoff roll, a/c dragged left wing on turf runway, and nearly collided with a camper. One fatality A/c climbed to between 100 and 150 feet above ground level, and made a "hard left turn" before it entered a nose-down descent and impacted terrain. Witnesses described the engine sound as "wide open" throughout the takeoff and accident sequence.
Oct 12, 2008 Kissimmee, FL CESSNA 172 N734BL A/c substantially damaged when impacted an athletic field shortly after takeoff from Kissimmee Gateway Airport (ISM). Witnesses said a/c began takeoff roll near threshold of runway 6, and became airborne prior to crossing runway15/33. One serious A/c began to lose altitude and drifted SE of runway. Pilot heard declaring an emergency on the ISM air traffic control tower (ATCT) frequency. One witness reported that a/c then "banked sharply to the left" and descended rapidly towards the ground.
Oct 16, 2008 Rock Hill, SC Hargett Sky Ranger II 75684 A/c substantially damaged when impacted a field. FAA inspector said no witnesses to accident. Pilot registered a/c in December 2007, but had not taken flight training, and was teaching himself how to fly. One fatality A/c impacted ground nose-down, and the tail was sticking up at an approximately 60-degree angle. Two composite propeller blades broken off at the hub. An estimated 3-4 gallons of fuel onboard a/c. No evidence of engine or airframe malfunction, and flight control continuity was confirmed from all flight control surfaces to the cockpit.
Oct. 21, 2008 Monroe, NC Experimental Quicksilver GT-500 Pilot was killed and passenger injured when ultralight crashed near a private airstrip northeast of Monroe. The plane crashed just before 1 pm near Arant Airport.
Oct. 22,2008 Grand Junction, CO Cessna 180 Mesa County Sheriff's Department plane carrying four people including two prisoners returned safely to airport following mid-air collision with plane carrying two people. No injuries No one injured in accident, including two who landed upside down in a field about 14 miles SE of airport. Cessna 180 was carrying two people and Cesna 210 collided 15 miles SE of Grand Junction.
