August 02, 2008 Fretin, France McDonnell Douglas MD-902OO-NHL A/c operated by Noordzee Helikopters Vlaanderen experienced a separation of the right hand sliding door while in flight. No injuries The flight originated from Lille Hospital, Lille, France, and was en route to Fourmies, France.
August 06, 2008 Salem, IA Grumman-Schweizer G-164B N48412 Accident occurred during aerial application flight. During course reversal engine "felt" as if it had no power. Pilot referenced his engine instruments and noted no anomalies. He assumed that a/c had encountered area of disturbed air, and elected to continue onto the next spray run. During next course reversal, engine started "popping" like engine misfire. No injuries Engine instruments referenced again and no anomalies. Pilot switched magnetos, verified that mixture was full rich, and selected full low pitch propeller setting. No change in engine performance. Pilot performed three-point landing in nearby grass field. A/c nosed ove during landing roll when encountered tall grass.
August 14, 2008 Jasper, TX Raytheon Aircraft Company C90A N688LL During initial takeoff climb at 300 feet agl at 140 knots, pilot noticed "buzzard" to front and slightly above the a/c. Pilot lowered pitch and right wing. However, the "bird folded his wings and dove," impacting right wing leading edge. Pilot "conducted controllability check" and noticed slight vibration with right aileron. No injuries Pilot continued to SE Texas Regional Airport (BPT), Beaumont, Texas, located 56 nautical miles from departure airport, because BPT had emergency services. The a/c landed uneventfully.
August 22, 2008 Moab, UT Beech A100 N601PC King Air impacted hilly terrain shortly after takeoff from the Canyonlands Field Airport (CNY). Witnesses did not see a/c take off, but did hear it, and takeoff sounded normal. About 15 minutes later, pilot that had been on telephone went outside and noted smoke in the distance. Ten fatalities Investigators went to accident site following day. A/c impacted on upslope side of hilly terrain in a right wing low nose high attitude about 1.2 miles SE of airport. A propeller blade from right engine propeller assembly was located in debris field near initial impact point. A/c came to rest on downslope side of hilly terrain and mostly consumed by post-impact fire. No evidence of in-flight structural failure found.
August 24, 2008 Yuma, CO Piper PA-24-235 N5476P A/c crashed one mile N of destination airport. Instrument meteorological conditions prevailed and flight plan not filed. Pilot and two passengers, fatally injured. Two witnesses heard a/c go over office "very low and very fast". Raced outside where could still hear a/c, but could not see it because of low clouds. Three fatalities One witness described hearing sound change as if a/c was starting to turn. Could barely make out a shape in fog, low to the ground and moving fast, when there was a loud "thump" and almost immediately he saw ball of fire.
August 27, 2008 Meeker, CO Air Tractor AT-802A N189LA single-engine a/c substantially damaged during impact with terrain following loss of control. unavail While making pass to drop fire retardant, a/c's cockpit access hatch opened. Unable to close hatch. Pilot elected to attempt another pass to jettison remaining retardant before returning to airport. During second pass a/c stalled and impacted terrain.
August 28, 2008 Las Vegas, NV Piper PA-31-350 N212HB A/c suffered partial loss of engine power during climb a few minutes after departing North Las Vegas Airport (VGT). As a/c returning to airport, collided with several objects and eventually a residence. Witnesses reported observing fire and white smoke in vicinity of a/c's right engine. NTSB investigator's review of FAA recorded radar data indicated that pilot took off from VGT (elevation 2,205 feet mean sea level), turned westbound while climbing. One fatality A/c passed abeam and north of VGT, and climbing through 3,400 feet. A/c five miles west of VGT and had climbed to 3,900 feet. A/c lost altitude during course reversal left turn, and descended through 3,200 feet. Pilot advised controller that "declaring an emergency" and desired to "immediately" return to runway 07 at VGT.
August 29, 2008 Ridgedale, MO Robinson R44 II N41411 A/c registered to Central Illinois Air Corp. crashed during cruise flight. A/c on a new aircraft delivery flight from Torrance, CA to Mattoon, IL, and had departed Shawnee Municipal Airport (KSNL) Shawnee, OK en route to intermediate stop at West Plains Municipal Airport (KUNO) West Plains, MO. Two fatalities Two witnesses watching the a/c in normal cruise when it descended slightly and then suddenly nosed over in attitude approximately 60 degrees nose down. After a/c passed from their view behind a ridgeline, heard a "boom" and saw a column of smoke.
August 29, 2008 Burbank, CA Cessna 172N N172ST A/c sustained substantial damage following forced landing near Burbank. One serious Airport operations personnel at Bob Hope Airport (BUR) revealed that pilot contacted ATC advising of a "fuel problem."
August 31, 2008 Angle Fire, NM Cessna 340A N397RA A/c substantially damaged when impacted terrain one mile E of Angel Fire Airport (AXX). Pilot said broke out of clouds and was "high." One minor Initiated circling approach and recalls turning back towards airport. Next event he remembers is waking up in his burning a/c. Pilot does not suspect mechanical anomalies with a/c.
August 31, 2008 Cleburne, TX Schleicher ASW-20B N30WK Glider substantially damaged upon impact with terrain. One fatality Spanish speaking eyewitness said glider crashed. FAA inspector documented crash site. Wreckage recovered to secure location for further examination.
August 31, 2008 Camarillo, CA Piper PA-18-150 N7482D A/c sustained substantial damage after impacting terrain while maneuvering at the Camarillo Airport (CMA). Banner tow flight. Witnesses said a/c approached banner tow pickup area at elevation of about 20 to 30 feet agl. One witness reported after hooking the banner the a/c pitched up to 45-degree angle before leveling off at about 200 feet above the ground. One fatality Witness stated that a/c flew straight and level 4 to 5 seconds, then suddenly nosed over to 30-degree angle before descending and impacting terrain. Another witness, a CFI, said a/c did not appear to stall, and nothing to indicate that a/c had entered spin prior to impacting terrain. No post-crash fire.
August 31, 2008 Oregon City, OR Wasson Wasson Loehle P-40 N510JW Experimental amateur built a/c substantially damaged following forced landing after experiencing loss of power. One serious Pilot reported that departed Aeroacres Airport (OG30), and after about 35 minutes engine surged twice then quited. Pilot reported that 300 yards from approach end of Runway 4, engine quit second time, which resulted in pilot making forced landing in private pond.
August 31, 2008 Greensburg, IN Bell 206L-1 N37AE A/c operated by Air Evac EMS destroyed during an in-flight collision with terrain and post impact fire. Witness saw a/c clear set of high-tension power lines E of departure point. Witnesses subsequently saw components separate from a/c in-flight before impact. Three fatalities Rotor blade/hub assembly separated at rotor mast and came to rest approximately 200 yards west-southwest of fuselage. Main rotor blades remained attached to hub. One blade remained intact. Other blade fractured into three sections, with inboard blade remaining attached to hub. Two separated blade sections were recovered at the accident site.
Sept. 01, 2008 Reno, NV Lockheed SP-2H N4235T A/c destroyed after impacting terrain following loss of power and loss of control two miles NW of Reno/Stead (4SD) Airport. A/c registered to Neptune Aviation Service, Missoula, MT, and operated by California Department of Forestry and Fire Protection. Air tanker base employee reported observing the a/c taxi to Runway 32...and everything appeared normal. Witness reported watching a/c takeoff, and at elevation estimated to be between 100 to 300 feet above the ground, observed left jet Three fatalities An air tanker base employee who witnessed the accident reported he engine emitting flames and left wing engulfed in flames. Initial onsite examination revealed that 500 feet from departure end of Runway 32, several identifiable pieces of a/c's left jet engine located. Also revealed that prior to impacting terrain the a/c had collided with set of power lines, estimated to be 50 feet high.
Sept. 1, 2008 Columbus, OH Convair 580 N587X A/c operated by Air Tahoma destroyed when impacted terrain as attempting to return to Rickenbacker International Airport (LCK). Flight was first flight following maintenance "C" check, which included flight control cable rigging as part of the check. Flight also intended to be training flight for first officer and company pilot seated in observer seat. Three fatalities Flight received takeoff clearance and started takeoff roll. About one minute later, pilot contacted ATC and stated that needed to return to LCK. Cleared to land on runway 5L. ATC asked pilot if needed equipment and the flight responded, "Negative." A/c impacted cornfield one mile SW of approach end of runway 5L.
Sept. 02 2008 Camarillo, CA Lancair Propjet N750PJ Single-engine amateur built a/c substantially damaged during forced landing following loss of engine power during climb after takeoff from Camarillo Airport (CMA). Two minor Pilot reported that during climb from runway 26, as a/c passed about midfield at altitude of 1,000 feet agl, engine lost power.
Sept. 04, 2008 Oahu, Hawaii USCG HH-65C Dolphin helicopter A/c crashed in waters off Oahu while conducting search and rescue drills with 47-f00t motor lifeboat. Three fatalities Witnesses aboard lifeboat and passing USAF C-17 saw crash.
