Oct 4, 2009 Gainesboro, TN PIPER PA-28-140 N95472 Pilot selected to depart from runway 36, a 3,500-foot-long, unlighted, asphalt runway. It was dark night, and pilot utilized "glow sticks" to illuminate points around runway. Pilot reported that takeoff and initial climb were normal. Did not have any further recollection of flight. One serious A/c impacted trees along rising terrain NW of airport. Witness said engine noise was "loud" with no fluctuations. Examination of a/c did not reveal any pre-impact malfunctions. Pilot received private pilot certificate about 3 months prior to accident and had 120 hours total flight experience
Oct 20, 2009 Montevideo, Uruguay MCDONNELL DOUGLAS MD11 N701GC Centurion Air Cargo a/c experienced right main landing gear failure during landing on runway 24 at Montevideo - Carrasco International Airport, No injuries Landing gear collapsed while taxing to ramp on taxiway A. The cargo flight originated from Miami International Airport, Miami, FL.
Oct 21, 2009 Sharjah, United Arab Emirates BOEING 707 ST-AKW Sudan Airways B-707 crashed shortly after takeoff from runway 30 at Sharjah International Airport, Sharjah, UAE. Six fatalities The intended destination of the cargo flight was Khartoum, Sudan.
Oct 25, 2009 Eupora, MS BELL 206 N57PH A//c operated by Provine Helicopter Service substantially damaged after forced autorotation following total loss of engine power. One serious Pilot had just refilled spray tank 90 gallons of herbicide and was conducting first turn at edge of land tract when a/c "completely lost power." Said rotor "stopped turning completely" 50 feet above round. A/c dropped "straight down" and landed on skids.
Oct 26, 2009 Norfolk, VA BOEING 717-200 N935AT A/c operated by Air Tran Airways not damaged when maneuvered to avoid another a/c during cruise-descent near Norfolk, VA. One flight attendant received serious injuries. One serious Pilots advised had been descending from flight level 350 to 330 due to turbulence when halted their descent "for an RA" (Resolution Advisory) from onboard terminal collision avoidance system (TCAS).
Oct 26, 2009 Minsk, Belarus HAWKER BAe-125-800B RA-02807 A/c operated by S-Air destroyed when impacted forested terrain during go-around attempt at Minsk-2 International Airport (UMMS), Minsk, Belarus. Five fatalities Three crew members and two passengers fatally injured. A/d was operating between Vnukovo Airport (UUWW), Moscow, Russia, and Minsk.
Oct 29, 2009 Christiansted, VI CESSNA 177 N3431T A/c destroyed when impacted field shortly after taking off from Henry E. Rohlsen Airport (TISX), Christiansted, St. Croix, U.S. Virgin Islands. Three fatalities Witness, a police detective, saw a/c take off to E and turn towards N.. It appeared to be "straining to gain altitude," while "flying low at a straight angle." Said "the wings began moving up and down," and the propeller was spinning. A/c then "suddenly" made sharp left turn and went down."
Oct 30, 2009 Miami, FL BERIEV A/C Co. BE-103 N13KL A/c substantially damaged during water landing in Biscayne Bay near Miami, FL. A/c designed and manufactured in Russia. According to manufacturer's web site, a/c was multipurpose amphibious monoplane with low-set water-displacement wing with a root strake, all-moving horizontal tail located in the propeller blow zone, and tricycle landing gear. No injuries Pilot decided to demonstrate water landing. On touchdown, right side of a/c "dragged," and the left wing of the airplane subsequently struck water abnormally. Caused "abrupt heading change of 60 degrees," and a/c stopped abruptly but remained afloat.
Oct 30, 2009 Las Vegas, NV Hallbauer Dragonfly Mark II N555RX A/c collided with runway light during takeoff from North Las Vegas Airport and subsequently crashed into an automobile. One serious A/c reached 200 feet agl and made right bank. Inspector noticed that piece of airplane's canard resting near runway light.
Nov 04, 2009 Tallahassee, FL CESSNA 172S N5194X A/d operated by Eagle Aircraft destroyed after collision with wooded terrain during forced landing at Tallahassee Regional Airport (TLH), A/ at 1,000 feet and three-fourths of a mile from runway, and one-half mile from perimeter fence when it began its descent. Two fatalities Tower controller tracking a/c visually, and noticed that landing light and position lights illuminated. Perceived the descent immediately, and voice communications revealed he transmitted a/c's registration number over radio. A//c responded with, "N5194X" in clipped, business-like manner. No further communications from a/c.
Nov 04, 2009 Show Low, AZ BEECH C-99 N330AV A/c encountered bird strike while on approach to Show Low Regional Airport (SOW). Pilot said shortly after beginning descent at altitude of 11,000 feet msl, 20 miles W of Show Low, bird impacted upper part of captain's windshield, breaking football size hole in it. Considerable amount of blood, tissue matter, and windshield fragments came into cockpit. One minor Captain suffered facial lacerations, bruising, and some lacerations on chest. Pilot continued approach to SOW in spite of fact windscreen was nearly opaque. Pilot made radio calls in the blind using standby hand microphone. Unable to hear any transmissions due to wind noise in cockpit.
Nov 04, 2009 Globe, AZ EUROCOPTER AS350BA N617LH A/c landed hard during Night Vision Goggle training flight at San Carlos Apache Airport (P13), Omniflight Helicopters, operating the a/c/ Two serious Pilots' said during NVG initial training flight, while performing simulated hydraulics failure, lost control of a/c before impacting ground. Pilots reported no in-flight abnormalities with a/c prior to initiation of simulated hydraulic failure.
Nov 06, 2009 Agnos, AR ZENITH AIRCRAFT ZODIAC 601 N538CJ Experimental light sport ai/c was destroyed when impacted terrain, following in-flight breakup 1.5 miles south of Agnos, AR. One fatality Debris field scattered over 600 feet. Right wing assembly initiated debris field, coming to rest in pond. Left wing assembly came to rest 200 feet from right wing.
Nov 07, 2009 Bonham, TX WICKS AIR CENTER HORNET N651WA Impacted ground one mile N of Jones Field Airport (F00). Non-pilot witnesses near the accident scene described weather as foggy and cloudy with restricted visibility. Two fatalities Witnesses saw a/c flying at low altitude in or near clouds when it suddenly pitched nose down and crashed.
Nov 07, 2009 Mount Victory, OH LOCKWOOD AIRCRAFT AIR-CAM N32786 A/c sustained substantial damage when impacted ground while landing at Elliot's Landing Airport (O74), One fatality Pilot, sole occupant, fatally injured. Flight originated from London, Ontario, Canada, at an undetermined time and was landing at the time of accident.
Nov 07, 2009 Adelanto, CA PIASECKI/PIKE PV-18 N183YP A/c collided with high tension wires and crashed near Adelanto, CA. Witness flying behind accident a/c in Bell 47 were talking on radio to each other. Watched the PV-18 lift off from airport and head southbound. Three fatalities He saw a/c flying southbound but it stopped climbing and stayed low to ground. Saw power lines ahead and tried to warn accident pilot but received no reply.
Nov 08, 2009 Spring Branch, TX BEECH A36 N91TD A/c departed controlled flight and impacted ground one mile NW of Kestrel Airpark Airport (1T7). A/c in cruise flight at 9,000 feet in an area of heavy rain when pilot requested and received permission for deviation around weather. Three fatalities Air traffic controller observed a/c in right turn and rapid descent and then lost radar contact with a/c Several witnesses reported hearing loud noise and seeing pieces of a/c falling to ground.
Nov 08, 2009 Altus, OK MOORE SKYBOLT/BM N6644 A/c destroyed when impacted terrain shortly after taking off from Quartz Mountain Airport (AXS). Two fatalities Witnesses said a/c started takeoff roll on runway 17. Pilot raised tail and when a/c 200 feet down runway, "yanked" a/c off ground, climbed 50 to 75 feet, then executed hard (90 degree) right bank. A/c then made wing-over and spiraled to ground.
Nov 14, 2009 Burnet, TX AUGUSTINE DAVID L SONEX N10DA Experimental a/c substantially damaged when impacted terrain one mile N of Burnet Municipal Airport - Kate Craddock Field (KBMQ). One fatality Pilot said having engine problems and attempting to land on runway 01 at KBQM. Witnesses said a/c cleared trees at approach end of runway and then nosed over, descending in nose low attitude. A/c impacted terrain between two rows of trees.
