Flight 491's Faulty Parts

The Transportation Safety Board found that the Sikorsky S-92A offshore crash in March, off the coast of Newfoundland, that killed 17 people, was due to a faulty tail rotor and the loss of its inflation collar system. Without the tail rotor, pilot control was at a minimum and when the crash occurred the flotation collars did not release its compressed gas, causing the aircraft to sink instead of remaining buoyant. The TSB is still investigating why the collars failed to inflate. Also, officials found two titanium mounting studs that attach to the main gearbox has cracked before the helicopter crashed.
