NTSB Preliminary Report Suggest Pilot Error in Fatal EMS Crash

A National Transportation Safety Board preliminary report says the Air Angels EMS helicopter that crashed into a radio tower near Chicago had no mechanical malfunctions, providing the strongest evidence so far of pilot error.
