In a just-released report, the U.K.'s Air Accidents Investigation Branch (AAIB) cited a recent incident as a major example of the need for better crew training, regarding low fuel management. The 2005 case involves a four-engine B747, which continued on a flight from Los Angeles to London on three engines, following an engine failure immediately after take off. The British Airways pilots' decision to press on to London after experiencing the fiery engine surge was sharply criticized by the FAA. The AAIB recommended that the U.K. Civil Aviation Authority and the FAA review the policy of flight continuation under such circumstances and that clear guidance be given airlines. More
